What are the issues with extensive restorations?
-pulp protection
-retention and resistance -restorative technique (direct/indirect) |
What happens to pulp if remaining dentin is less than 1mm?
-odontoblast viability decreases
-pulpal inflammation increases |
What limits retention occlusally and laterally?
lack of opposing walls
What are some retention considerations?
pins and posts
-extended outline- dovetail -extended internal form |
What are the cons to pins and posts?
-tend to weaken tooth and restorative material
-more for flat 1 surface cavity -placed near line angle -.5-1mm from DEJ, parallel to external surface |
What are some extended internal form considerations?
-vertical grooves in opposing walls
-coves -slots -amalgapin |
What are the parameters of an amalgapin?
1-2mm deep channel prepared in dentine
1mm wide, bevelled orifice |
What are the resistance considerations?
-weakend unsupported cusps
-cusp overlay, material thickness, positioning of margins |
Direct Vs. indirect
What are some examples of indirect restorations?
-inlay, onlay
-crown |
How do you do a stainless steel crown for decid tooth?
-caries removal
-occlusal reduction- 1-1.5mm with high speed bur -proximal reduction- high speed bur, break contact on gingival, buccal and lingual -explorer should be able to pass through entire contact -buccal and lingual reduction -round line angles, round occlusal 1/3 of buccal/lingual surfaces, cervical buldge retention |
what do you do is SSC is too long?
trim with scissors following contour of gingival margin, smooth margins with stone or wheel
How do you seat SSC?
-seat buccal to lingual, slide crown over buccal cervical buldge
Why might the SSC not seat well?
-proximal contacts not broken
-gingival ledge -inadequate occlusal reduction -SSC too long |
What do you cement on with?
GIC, 1/2 to 2/3 fill crown
once cemented what do you check?
-marginal ridge heights match adjacent teeth
-proximal contact restored -margins closely adapted, extend into sulcus may blanch -remove all cement left over |
