pain
pgf2alpha
pge2
NGF
What does PGF2alpha do?
- vasoconstrictor involved in uterine contractions
- dysmennorrhea & abnorma contractions during menstruation
pge2
- produced by inflammatory cells & endometriotic stromal cells
- activates nerve fibres causing pain
- promotes estrogen biosynthesis
NGF
- promotes growth sensory, sympathetic and parasympathetic nerves
- present in eutopic endometrium of women with endometriosis but not normal women
- unmyelinated c fibres found exclusively in functionalis of women with endometriosis
mechanisms leading to infertility
- inflammatory products are toxic to gametes and embryos
- fibriae malfunction to due to fibrosis
- dysregulated progesterone regulated genes in endometrial stroma prevents implantation
epigenetic effects on gene function
endometric lesions
- less than normally methylated increasing expression levels compared to endometrium e.g. ERalpha.
- micro RNAs are altered
epigenetic effects on gene function
eutopic endometrium
- several genes involved in implantation are HYPERMETHYLATED
epigenetic effects on gene function
endometriosis
DNA methyl transferases are upregulated suggesting a widespread genome methylation
Why do some women develop endometriosis?
GENETIC PREDISPOSITION
genes involved in endometrial development
estrogen receptor-a regulation
epithelial cell adhesion
