Indic- unstable angina, acute myocardial infraction

Class- anticholinergic(parasympathetic blocker)
Action- Direct vagolytic action increasing sinus node automaticity and faster condution through the AV node
onset- 1-5 min
Indic- Symptomatic bradycardia, asystole, bradycardic(<60) pulseless electrical activity (PEA)
Cont- Angle-closure glaucoma, tachycardia
side effects- Headache, hot flushed skin, disorientation, tachycardia, dry mouth, blurred vision
Route- IV,IO,ET/ vials
Dose- Symptomatci bradycardia AD: 0.5-1.0mg rapid repeat 3-5min max 0.04mg/kg
Asystole or bradycardic PEA
AD: 1.0mg IV or ET repeat 3-5 min max 0.03-0.04mg/kg
Organophosphate poisoning AD: 2mg IV or IM 5-15 min repeat till secretions clear.
Ipratropium Bromide

Class- anticholinergic
Act- inhibits interation of acetylcholine at muscarinic receptors sites. results in bronchodilation and drying of secretions
Onset- 10-15 min
Cont- atropine, peanuts, soybean proten
route- MDI or nebulizer
dose- MDI:2 inhalations (36ug) Neb: 0.5mg in 3.0ml saline
Ped: <12 0.125-0.250mg >12 not recommended
Albuterol Sulfate
Albuterol, Proventil, ventolin
Clas- sympatho minmetic, bronchodilator
act- selective beta-2 adrenergic stimulation
Onset- 5-15min
Indic- Bronchospasm associated with asthma, emphysema, or allergic reation.
Con- history of seizures, diabetes, hypertyroidism
route- nebulizer MDI
Dose- MDI: 90ug up to 3
Neb: 2.5mg in 3ml salution repeat 15min
Ped Neb: >12 0.03ml/kg up to 1ml
Nitroglycerin (NTG)
nitroglycerin, nitrostat, Nitro-bid
Class- vasodilator
Action- Decreases pain of ischemia, benous blood return to heart, dilates coronary arteries
Onset- 2-5min
Indic- Angina pectoris, suspected acute myocardial infarction
Conter- Closed-angle glaucoma, head traum systolic bp <90 use of viagra, cialis, or levitra
Route- Sub ligual
dose- tablets .3,.4 and.6mg or metered spray.4mg/ twice for both tab and spray

class- carbohydrate (hypertonic solution)
Action- increases blood glucose levels
Onset-1min
Indications- hypoglycemia, ALC, coma of unknown cause, seizures of unknown cause
Contra- intracranial pressure, s/troke, delirium tremens

Action- binds to opiate receptor sites

dose- AD: 0.4-2.0Mg over 2-3 min up to 10 mg
Ped: >5 0.1mg/kg
1Lidocaine HCI 2%
Lidocaine, Xylocaine
Class- Antiarrhythmic
Action- Sodium Channel blocker; decreases automaticity, increases ventricular fibrillation (VF) threshold and electrical stimulation threshold of ventricle during diastole
Onset-1-5min
Indic- VF or pulseless ventricular tachycardia (VT)
Cont- Bradycardia (<60 beats/min)
Side effects- Tachycardia, nystagmus, labile blood pressure, respiratory depression, slurred speech, cardiovascular collapse
Route- IV or ET; rapidly
Dose- Cardiac arrest (VF or pulseless VT)- AD: 1.0-1.5 mg/kg repeated every 5-10 min max 3.0mg/kg
IV infusion following resusciation- AD: 1-4mg/min (30-50ug/kg per minute. can dilute in D5W or normal saline

Class- Benzodiazepine
Action- Suppresses seizure activity across cerebral motor cortex
Onset IV 1-5min IM: 15-30
Indi- Sedation or treat siezures
contra- Acute angle-closure glaucoma, shock, repiratory depression
side effects- sedation, ataxia, hypotension. possible respiratory depression
Route- IV or IM slowly
Dose- AD: 2-10mg over 5-10 min max 30mg
ped: up to 5 0.2-0.5mg every 2-5min max 5.0mg
Epinephrine hydrochloride 1:1,000
Epinephrine/ adrenalin-
Class-sympathomimetic
Action- Alpha and beta agonist; increases systemic vascular resistance, relieves bronchospasm
Onset- SC: 5-10 min
Indic- mild to moderate asthma and allergic reactions
Countr- cautions with hypertension, hyperthyroidism, angle-closure
Side Effects- Headache, restlessness, palpitations, tachycardia, anginal pain, dysrhythmias
route- SC;slow
dose- adult: 0.3-0.5mg Ped: 0.01mg/kg max 0.3mg
Terbutaline
Terbutaline/Brethine,Brethaire/ Class- Bronchodilator
Action- B-2 adrenergic stimulation resulting in relaxation of bronchiole smooth muscle
onset- 5-15min
Indi- asthma, chronic bronchitis, and emphysema
contra- caution history of seizures, diabetes, hyperthyroidism
side effect- tremors, PVC, palpitations, flushing sweating, increased heart rate &BP, dizziness, nausea, headache
route- SC or MDI
dose- AD:MDI:1-2 inhalations of 0.2mg per every 4-6 h (1min between)
SC:0.25mg every 15-30min as needed
not for under 12y
Adenosine
Adenosine/Adenocard-
Class- Antiarrhythmic
Action- Slows conduction through AV node. Inhibits reentry through the AV node
Onset- 10-15sec
Indic- Conversions of paroxysmal supraventricular tachy and supraventricular tachy to reg sinus rhythm
Cont- 2nd 3rd heart block, sick sinus rhythm
Side effect- Asystole or bradycardia, ventricular ectopy, dyspnea, flushing, chest pain, dissiness, metallic taste, apprehension
route- IV or IO: rapidly
dose- 6mg followed 20cc saline no response in 1-2min repeat at 12mg up to 2 X totoal=30mg
Morphine sulfate(MSO4)
Morphine/Duramorph,AstromorClas: Narcotic
Action- Opiate receptor agonist, reduces pain of myocardial ischemia
onset- 1-2min
Indicaction-Pain management, ie burns or myocardial infarction; pulmonary edema
con- acute bronchial asthma, CNS depression, head injury
6.dizziness, hypotension, bradycardia, respiratory depressioni, undiagnosed abdominal pain.
route- IV slow
dose- Ad: 2-4mg over 1-5min to effect ped; 0.1-0.2mg/kg max, 15mg
