Acetaminophen


(Tylenon, APAP)

Analgesic/Antipyretic. Reduces fever by acting directly on the heat regulating center of the hypothalamus.


Indications: Fever


Dose: Adult 650-1000 mg PO, Pedi 15mg/kg PO

Adenosine


(Adenocard)

Slows tachycardias associated with AV node. Acts on sinus pacemaker cells and vagal nerve to decrease chronotropic and dromotropic activity.


Indications: PSVT/SVT


Contraindications: 2nd or 3rd degree block, sick sinus, caution in pregnancy and pts with wheezing.


Dose: (adult)12mg rapid IV/IO, repeat 12mg

Albuterol


(Ventolin, Proventil)

Adrenergic beta 2 agonist, sympathomimetic, bronchodilator. Stimulates beta 2 adrenergic receptors in lungs and causes a relaxation of bronchial smoothe muscle.


Indications: Relief of bronchospasm, resp distress, allergic reaction, hyperkalemia


Dose: 2.5mg with non humidified oxygen at 8-10 L/min

Amiodarone


(Cordarone)

Class III anti-arrythmic. Acts directly on the cardiac cell membrane by blocking sodium channels, prolongs repolarization and refractory period. Acts on peripheral smooth musc


Indications: V-Fib, hemodynamically unstable V-Tach, narrow complex tachycardia


Contraindications: caution with pt's with hypokalemia and hypomagnesemia, heart block


Dose: V-Fib 300 mg, V-Tach 150 mg

Aspirin


(Acetylsalicylic acid, ASA)

Analgesic, antipyretic, and anti-inflammatory, inhibits platelet aggregation.


Indications: Ischemic chest pain


Contraindications: previous reactions, current anticoagulant therapy, suspected active hemmorage


Dose: 160-325 mg tablets

Ativan


(Lorazepam)

CNS depressant (sedative) with muscle relaxant and anticonvulsant properties, used to relieve anxiety and tension, seizures.


Indications: sedation for acute stress, acute alcohol withdrawal for DT or agitation, recurrent convulsive seizures, sedation before cardioversion


Contraindications: alcohol or sedative ingestion, resp depression, shock, coma, isolated musculoskeletal injuries.


Dose: 0.5 to 2 mg

Atropine


Autonomic nervous system inhibitor, blocks parasympathetic action.


Indications: sinus brady with PVCs, hypotension, 2nd degree or 3rd degree AVB with bradycardia, organophosphate poisoning


Contraindications: Atrial flutter or A-Fib, h/x of glaucoma.


Dose: bradycardia 0.5 - 1mg, organophosphate poisoning 2mg

Atrovent


(Ipatropium Bromide)

Anticholinergic bronchodilator.


Indications: resp distress due to asthma, chronic bronchitis, emphysema


Contraindications: Caution with CAD, hypertension, hyperthyroidism, and diabetes.


Dose: 0.5 mg in 2.5 mL

Benadryl


(Diphenhydramine)

Antihistaminic agent, produces anticholinergic and sedative effects.


Indications: Anaphylactic shock and severe allergic reaction, dystonia, motion sickness


Contraindications: Pregnancy, Asthma, Glaucoma, alch/depressant abuse


Dose: 50mg IV/IM/IO

Cardizem


(Diltiazem)

Calcium channel blocker


Indications: narrow complex tachy refractory to adenosine, a-fib/a-flutter.


Contraindications: V-Tach, sick sinus, 2nd or 3rd degree AV block


Dose: 20 mg IV

Calcium Chloride

Cation Electrolyte.


Indications: mag sulfate or calcium channel blocker OD, cardiac arrest in renal dialysis patients, arrhythmias due to hyperkalemia, relieft of pain and muscle spasms due to black widow bite


Contraindications: caution in digitalis, hypercalcemia, vfib, kidney stones, CHF.


Dose: 1.0 grams

Dextrose 10% - 25% - 50%

Hypertonic dextrose solution.


Indications: Hypoglycemia, uncon of unk origin, seizure of unk origin


Contraindicaitons: presenting with S/S of CNS pathology, intracranial hemorrhage, cerebral edema, increased ICP


Dose: 25 grams IV/IO

Dopamine


(Inropin)

Acts as a beta sympatheic mediator, exerts an inotropic effect on the mycardium, dilates renal vasculature.


Indications: hypotension during non-hypovolemic shock, symptomatic bradycardia


Contraindications: not to be administered in presence of uncorrected tachyarrhythmias or VFib, acute hypertension.

Ephinephrine


(Adrenaline)

Endogenous catecholamine with nonselective beta1 and beta2 and alpha properties


Indications: Anaphylaxis / acute allergic reaction, asthma, bradyarrhythmias resistant to atropine.


Contraindications: None in cardiac arrest, hypovolemic shock.


Dose: Cardiac arrest- 1mg IV/IO, Allergic reaction- 0.3 mg IM, Anaphylaxis- 0.5 mg IM, Bronchospasm- 0.3 IM

Etomidate


(Amidate)

Non-barbituate hypnotic.


Indications: induction of anesthesia prior to administration of neuromuscular blockade.


Dose: 0.15 mg/kg IV

Fentanyl


(Sublimaze)

Potent synthetic with analgesic properties.


Indications: Control of moderate to severe pain


Dose: 1mcg/kg IV/IO

Glucagon

Pancreatic hormone-insulin antagonist, causes breakdown of glycogen to glucose.


Indications: Altered LOC where hypoglycemia is suspected and an IV route is not immediately available, beta blocker OD, calcium channel blocker OD


Contraindications: Allergy to proteins


Dose: 1mg IM


Glucose, oral

Carbohydrate, anti-hypoglycemic.


Indications: Hypoglycemia with altered mental status.


Contraindications: Inability to swallow.


Dose: 15-45 grams PO

Haldol


(Haloperidol)

Antipsychotic, neuroleptic, blocks dopamine receptors.


Indications: used in acute psychotic episodes, may use with Ativan especially in sympathomimetic OD (cocaine, methamphetamines)


Contraindications: children under 14


Dose: 5-10 mg

Ibprofen


(Motrin)

Nonsteroidal anti inflammatory drug with signifigcant antipyretic and analgesic properties.


Indications: Fever


Contraindications: hypersensitivity, peptic ulcer disease, bleeding abnormalities, renal disease, hepatic disease.


Dose: 600mg PO

Ketamine Hydrochloride


(Ketalar)

CNS agent produces a rapid-acting dissociative effect with limited cardiovascular or respiratory side effects.


Indications: Rapid control of the agitated/exited delirium patient requiring medical intervention


Contraindications: Hypertensive crisis/Angina/Chest pain/MI, airway management.


Dose: 20mg dose, secondary to Fentanyl.

Magnesium Sulfate

Electrolyte salt solution with CNS depressant like properties.


Indications: Refractory VFib or VTach, Torsade-de-point, Seizures with eclampsia, bronchospasm


Contraindications: High degree AV block


Dose: 1-2 grams IV/IO

Narcan


(Naloxone)

Synthetic opioid antagonist.


Indications: known narcotic OD, Uncon of unk etiology.


Contraindications: Caution in narcotic-dependent patients who may experience withdrawl


Dose: 0.4-2 mg

Nitroglycerin


(Nitrostat, Nitrolingual Spray)

An organic nitrate, smooth muscle vasodilating agent.


Indication: Ischemic chest pain, CHF


Contraindication: Caution in patients presenting with hypersensitivity, ICP/head injury, hypotension.


Dose: 0.4 mg

Nitrous Oxide


(Nitronox, NO2)

Analgesic, olorless gas that acts on the CNS.


Indication: moderate to severe pain, extremity trauma, acute MI, or burns.


Contraindications: Altered LOC, alcohol ingestion or drug OD, Pregnancy


Dose: Self administered through inhalation

Rocuronium

Non-depolarizing neuromuscular blocker.


Indication: To facilitate paralysis during RSI.


Contraindication: Inability to ventilate.


Dose: 1.0 mg/kg

Sodium Bicarbonate

Hypertonic solution used as a systemic alkalizer, reacts with hydrogen ions to form water and carbon dioxide and thereby can act to buffer metabolic acidosis.


Indication: Cardiac arrest situations with a down time greater than 10 minutes, TCA OD, known preexisting bicarb-responsive acidosis.


Contraindication: None, in life threatening emergencies, CHF, Metabolic and respiratory alkalosis.


Dose: 1mEq/kg IV/IO

Solumedrol


(Methylprednisolone)

Synthetic steroid that suppresses acute and chronic inflammation, beta adrenergic agonist


Indications: Bronchial asthma and other bronchospastic states, life threatening shock states.


Contraindications: Caution in patients with GI hemorrhage and diabetes mellitus, fungal infection.


Dose: 125 mg slow IV

Thiamine

Vitamin B complex.


Indications: patients presenting with s/s of alcohol toxicity, Wernicke's or Korsakoff's phychosis, uncon pt of unk etiology. Thiamine should be administered prior to D50 and Narcan.


Dose: 100 mg IV/IO

Zofran


(Ondansetron)

Blocks the actions of chemicals in the body that can trigger nausea and vomiting.


Indications: Prevention of nausea and vomiting


Contraindications: Caution in liver impairment, abdominal surgery, pregnancy.


Dose: 4.0 mg over 2 min
