Frostnip
Also called grade I frostbite - no persistent deficit after rewarming, no blisters; just very painful and erythematous; does not require admission. Does not lead to amputation
Meds
ASA or NSAIDs
tetanus toxoid
PCN for high grade disease
narcs as needed
Role of NSAIDs/ASA
Inhibits Thromboxane, a large part of blister contents. Thromboxane mediates tissue damage.
Time course
Blisters appear within 24 hours after injury; Takes up to 3 months to tell whether tissue will survive or mummify (requiring amputation).
Radiography
Technetium scan on day 2 can predict need for amputation - if how or no tracer, will probably need amputation
