Adult: 2 to 20 mcg/kg per minute @ dilution ratio of 250mL, 500mL, 1000mL of NS or D5W (or Lactated Ringers depending on fluid requirements of the pt.

Pedi: mgc/kg titrated to desired effect

Dilution ratio: 16:6
Dopamine HCL
Adult: 2 to 20 mcg/kg/minute (BioTel)
Dilution ratio: Use a 400 mg/250mL D5W (premix) or add 400 mg to 250mL NS.
OR
Use 800 mg/500mL D5W (premix) or add 800mg to 500mL NS.
After BioTel determines mcg/min, refer to the Drug Drip Chart for drops/minute.
Pedi: not used in prehospital setting
Lidocaine
Adult: V-Fib, unstable V-tach or pulseless V-tach - 1.5 mg/kg.
Non-sustained v-tach or treatable PVC's 1mg/kg
Max. total dose 3mg/kg

Pedi: 1mg/kg
Magnesium Solfate
Adult: CPR for Torsades de Pointes - 2grams of a 10 percent solution infused IV push over 6 to 10 minutes.

V-tach (torsades de pointes with a pulse) 2 grams in 250 mL NS infused IVPB over 6 to 10 minutes.

Refractory Bronchospasm: 2 grams in 250 mL NS infused IVPB over 6 to 10 minutes.
(not used for pediatrics in prehospital setting)
Norepinephrine (Levophed)
Adult: 8 to 10 mcg/minute "Recipe" (dilution ratio) - add 4mg to 250 mL NS (16 mcg/mL)
OR
Add 8 mg to 500 mL NS (16mcg/mL)

After biotel determines mcg/minute, refer to drug drip chart for drop/minute.
Not used for pediatrics in prehospital setting.
Procainamide Hydrochloride
Adult: 20 mg/min IV piggyback (maximum total dose of 17mg/kg)
50mg/min IV push in urgent situations (maximum total dose of 17 mg/kg)

Stop the medication when one of the following occurs:
Max. dose of 17 mg /kg or bag is empty)
Arrhythmia is suppressed
QUS widens by greater than 50%
Hypotension occurs
Sodium Bicarbonate
Adult: 1 mEq/kg IV push

Pedi: same
Glutose
Adult: Full Tube of 25 to 50 mg given in small doses.

Pediatric: 0.5 gm/kg given in small doses
Glucagon
Adult: 1 mg (may be repeated in 15 minutes.

Pediatric: (May rarely be ordered by biotel). -over 1 year old=1mg
Under 1 Year = 0.5 mg
Dextrose 50
Adult: 25-50 grams of D50
Pediatric: 0.5 gm/kg
Newborn to 1 month: D10 Dilute D50 (1:4) and administer 5mL/kg
1 month - 12 years - D25 Dilute D50 (1:1) and administer 2 mL/kg
Adenosine
12 mg rapid IV push followed immediately by a rapid 20mL NS flush.
Repeat doses (if indicated) are the same.
Pediatric: contact biotel.
Midazolam (Versed)
0.1 mg/kg max 5 for seizures
Diazapam (Valium)
2.5mg up to 10 for seizures
Adenosine
12 mg rapid IV followed immediately by 20mg NS rapid flush.
Dextrose
25 g IV followed by another 25 g if necessary.
Diltiazem (Cardizem)
First Dose 0.25 mg/kg IV Push over 2 minutes
Second Dose given 15 minutes after first dose is 0.35 mg/kg
Diphenhydramine HCL (Benadryl)
25 - 50 mg
Glucagon
1 mg may be repeated in 15 minutes.
Glutose
25 to 50 mg tube
Naloxone (Narcan)
0.4 titrated to effect
Ondansetron HCL (Zofran)
2mg slowly IV or IM (do not exceed 4 mg cumulative dose.
