BUTALBITAL, APAP, CAFFEINE
BRAND: Fioricet

CLASS: Barbiturate

INDICATION: Relief of the symptomatic complex of tension or muscle contraction headache

DOSING: PO 1-2 tablets or capsules (or 15-30mL soln) q4hrs
***Don’t exceed 6 tabs/caps (180mL soln) daily!

CONTRAINDICATIONS: Hypersensitivity to any of the components, porphyria (deficient heme); apap allergy/hypersensitivity, aromatic anticonvulsant allergy/hypersensitivity

Side effects: Dizziness, drowsiness, lightheadedness, sedation; abd. pain, N&V; dyspnea; intoxicated feeling
CODEINE/APAP (c-iii)
a. Brands: Capital + Codeine, Tylenol + Codeine No.3 (and 4)
b. Class: Analgesic, Opioid
c. Indications: Relief of mild to moderate pain
d. Dosing:
i. Antitussive – PO codeine 15-30mg/dose q4-6hrs (max. 360mg/day based on codeine)
ii. Analgesic – PO codeine 30-60mg/dose q4-6hrs (max. 4g/day based on apap component)
1. 1-2 tablets q4hrs (max. 12 tablets/24hrs)
iii. Don’t exceed >60mg in general!
e. Contraindications: Hypersensitivity to any of the components, significant resp. depression, acute/severe bronchial asthma, hypercapnia (too much CO2 in blood), paralytic ileus; apap or opioid allergy/hypersensitivity
f. Warnings: Use caution in pts w. hypovolemia, CV disease, or drugs which may exaggerate hypotensive effects (ie. phenothiazines or general anesthetics), hypersensitivity rxns to other phenanthrene-deriv. opioid agonists (ie. hydrocodone/morphone, levorphanol, oxycodone/morphone), adrenal insufficiency (inc. Addison’s disease), CNS depression or coma, history of drug abuse or acute alcoholism, alcoholic liver disease, G6PD deficiency, head injury, intracranial lesions, or elevated intracranial pressure, severe hepatic/renal impairment, pre-existing resp. probs, history of seizure disorder, thyroid dysfunction
g. Side effects: Dizziness, lightheadedness, sedation; N&V; dyspnea
PROPOXYPHENE/APAP (c-iv)
a. Brands: Balacet 325, Darvocet A500, Darvocet-N 100, Darvocet-N 50
b. Class: Analgesic combination (opioid)
c. Indications: Management of mild to moderate pain
d. Dosing:
i. A500/-N100 – 1 tablet q4hrs prn (max. 600mg propoxyphene napsylate/day)
ii. –N50 – 1-2 tablets q4hrs prn (same max as above)
iii. Propoxyphene HCl 65mg + APAP 650mg – 1 tablet q4hrs prn (max. 390mg/day pro.)
e. Contraindications: Hypersensitivity to any of the components, pts w. paralytic ileus, acute/severe asthma, hypercarbia (too much CO2 in blood), or severe resp. depression (unless mechanically ventilated); apap or opioid allergy/hypersensitivity
f. [Black Box Warning] – Accidental & intentional overdose occurred when propoxyphene used alone or in combo w. other CNS depressants inc. ethanol. Pts at risk are those w. history of emotional disturbances or suicidal thoughts/attempt or those on concurrent sedatives/tranquilizers/muscle relaxants/antidepressants/other CNS-depressants
i. Also use caution in pts taking strong CYP3A4 inhibitors!
g. Warnings: Use w. caution in pts w. biliary tract dysfunction, CNS depression or coma, history of drug abuse or acute alcoholism, alcoholic liver disease, known G6PD deficiency, head injury/intracranial lesions, etc., hepatic/renal impairment, pre-existing resp. probs
HYDROCODONE/APAP (c-iii)
a. Brands: hycet, Lorcet 10/650, Lorcet Plus, Lortab, Margesic H, Maxidone, Norco, Stagesic, Vicodin (ES, HP), Xodol (10/300, 5/300, 7.5/300), Zamicet, Zydone
b. Class: Analgesic Combination Opioid
c. Indications: Relief of moderate to severe pain
d. Dosing: PO hydrocodone 5-10mg qid
i. Dosage Range – Hydrocodone 2.5-10mg q4-6hrs (max. 60mg/day)
e. Contraindications: Hypersensitivity to any of the components, CNS depression, severe resp. depression; apap or opioid allergy or hypersensitivity
f. Warnings: Similar to above drugs
g. Side effects: Bradycardia, cardiac arrest, circ. collapse, coma, HTN; anxiety, dizziness, fear, lethargy, malaise, mental clouding, mood changes, sedation, stupor; hypoglycemic coma; N&V, constipation, abd. pain; ureteral spasm, urinary retention, vesical sphincter spasm; skeletal muscle rigidity; hearing impairment or loss (w. chronic overdose); acute airway obstruction, clamminess, etc.
OXYCODONE/APAP (c-ii)
a. Brands: Endocet, Magnacet, Percocet, Primalev, Roxicet (5/500), Tylox
b. Class: Analgesic, Opioid
c. Indications: Management of moderate to severe pain
d. Dosing:
i. Mild to moderate pain – initial dose 2.5-5mg
ii. Severe pain – initial 10-30mg
iii. *doses should be given q4-6hrs prn
e. Contraindications: Hypersensitivity to any of the components, severe resp. depression, pregnancy; apap or opioid allergy or hypersensitivity
f. Warnings: Same as above drugs
g. Side effects: Allergic rxn, constipation, dizziness, dysphoria, euphoria, lightheadedness, N&V, pruritis, resp. depression, sedation, skin rash
