M agonist
Glaucoma, Pupil contraction, release of introcular pressure
Pilocarpine
M agonist
Potent stimulator of sweat, tears, saliva, contracts ciliary muscle (open angle), spincter (narrow angle). Pile on the sweat and tears.
Methacholine
M agonist
Challenge test for dx of asthma. Stimulates airway M receptors when inhaled.
Neostigmine
AchEi
Postop for ileus and urinary retention. MG, reverse neuromuscular junction blockade. No CNS. (NEO CNS)
Pyridostigmine
AchEi
Get RID of MG. No CNS. MG Does not affect the brain.
Edrophonium
AchEi
Dx MG, extremely short acting ~30mins.
Physostigmine
AchEi
Glaucoma, for the PHYES. Also PHYX atropine overdose because it enters the CNS.
Echotiophate
AchEi
Glaucoma.
Symptoms of cholinesterase inhibotor poisoning?
DUMBBELSS
diarrhea, urination, miosis, bronchospasm, bradycardia, excitation of skeletal muscle and CNS, lacrimation, sweating, salivation.
Antidote for AchEi poisoning?
Pralidoximine (2-PAM) plus Atropine. 2-PAM must be used within 2hrs. Chronic exposure (farmer), neuropahty, muscle weakness, sensory decline, demyelination from drug diffusion NOT AchEi effect.

Organophosphates, lipid soluable, CNS, irreversible, insecticides(malathion, parathion), nerve gas (sarin, sabin)
Atropine
M antagonist
Eye, produce mydriasis and cycloplegia
Benzotropine
M antagonist
CNS, for parkinson's. Park my BENZ.
Scopolamine
M antagonist
CNS, motion sickness, end of life nausea
Ipratropium
M antagonist
Respiratory, Asthma, COPD.
Oxybutynin
M antagonist
Urine Bladder, reduce urgency, relax bladder spasms.
Glycopyrrolate
M antagonist
Urine bladder, also respiratory, decrease secretions during surgery
Methscopolamine
M antagonist
GI, peptic ulcer treatment, decrease acid, GI motility, salivation
Propantheline
M antagonist
GI, peptic ulcer treatment, decrease acid, GI motility, salivation
Side effects of M blockers
Hot as a hare, Dry as a bone, Red as a beet, Blind as a bat, Mad as a hatter, Bloated as a toad.
Dilates pupil, paralyzes ciliary muscle, decrease secretions, decrease motility and urgency, increase body temp, rapid pulse, dry mouth, flushed skin, constipation
Who is at risk for atropine treatment? (or any other M blocker)
Men w/ BPH (narrow hole, harder to pee), acute angle glaucoma, febrile infants, alzeheimers pts, ileus or gi obstruction
Hexamethonium
Nicotinic antagonist
Ganglionic blocker, prevents vagal reflex responses to changes in bp. ie prevents reflex bradycardia by NE. SE: Severe orthostatic hypotension, blurred vision, constipation, sexual dysfunction.
