Tradenames: Humalog, insulin lispro
onset: 30-90 min.
Duration: 6 hours
Admin: S.C. only up to 15 minutes before a meal. Monitor blood sugar level freq.
Can be mixed but draw first to avoid dosage errors.
HUMALOG INSULIN

Intermediate acting insulin

Onset: 1-3 hours
Peak: 4 to 15 hours
Duration: 18-24 hr
Admin: S.C. injection only 30 min before meal or snack.
Rarely produces a BSL that is close to normal, expect to mix with short acting.

Indications- prevent & treat deep vein thrombosis & pulmonary embolism
ADULT DOSAGES: SC injection 5,000 U before surgery and then 5,000 U q 8-12 hr for 7 days until pt is fully ambulatory.
USE HEPARIN CAUTIOUSLY IN ALCOHOLICS, menstr.women,oer 60,mild hepatic or renal disease, hx of allergies, asthma, or GI ulcer
HEPARIN
Warfarin Sodium - Coumadin
Class & Category
Chemical: Coumadin derivative
Therapeutic: Anticoagulant
Indications: prevent pulmonary embolism, recurrent MI
ADULT DOSAGES:
Tablets Initial 2-5mg/day for 2-4 days. 2-10 mg based on target PT & INR results
IV Injection: Intial 2-5mg/day infused 1-2min and the 2-10mg/day infused 1-2 min. Max: determined by trget PT & INR INR LEVELS 2.0-3.0
Advoid IM injections during usage due to possible bleeding, hematoma, bruising.
