What drug class is Amiodarone
Antiarrhythmic
What are the pharmacological actions are Amiodarone
Amiodarone is a antyarrhythmic that prolongs the action potential and refractory period. It inhibits adrenergic stimulation. Slows the sinus rate, increases PR and QT intervals, and decreases peripheral vascular resistance.
What are the indications of Amiodarone.
VF/Pulseless Vtach
Wide complex tachycardia with a pulse |
What are the contraindications for Amiodarone
Hypersensitivity
Cardiogenic shock Bradycardia AV blocks Liver disease |
What are the precautions and side effects of Amiodarone
Hypotension
braducardia cardiogenic shock headache/diziness AV block |
What is the dose for Amiodarone.
-VF/Pulseless, V tach 300 MG IV/IO repeat at 150 mg after 3-5 minutes.
-Wide complex tachycardia: 150 MG IV/IO over 10 minutes - Post resuscitation maintanance: 1mg/min over 6 hours - Max Dose: 2.2g IV/IO over 24 hours |
What drug class is Magnesium Sulfate
Electrolyte
What are the pharmacology and actions magnesium sulfate
magnesium sulfate acts as a physiological calcium channel blocker, CNS depressant and anti convulsant. It depresses the function of smooth, skeletal and cardiac muscles
what are the indications for Magnesium Sulfate
Hypomagnesaimia
Tosades de pointes eclamptic seizure adjunctive treatment in moderate to sever acute asthma |
what are the contra-indications for Magnesium Sulfate
Hypocalcemia
heart block myocardial damage anuria |
Precautions and side effects for Magnesium Sulfate
ALOC
Hypotension Heart block Hypocalcemia |
What are the dosages for Magnesium Sulfate
-Torsades de pointes 1-2G IVP over 1-2 minutes
-Eclamptic Seizure 4-5G IVP currently with up to 5 G IM in each buttock, then 4-5g every hour -Bronchodialation : 2G IVP |
What are the classes of Pralidoxime (2PAM)
Cholinesterase reactivator
What are the pharmacological actions for Pralidoxime (2PAM)
Reactivates the cholinesterase inhibited by phosphate esters by displacing the enzyme from its receptor sites. This restores the normal functions of neuromuscular transmission
What are the indication Pralidoxime (2PAM)
Organophosphate poisoning
Contraindications of Pralidoxime (2PAM)
Severe cardiac disease
peptic ulcer asthma myocardial infarction hypersensitivity to procainamide/procaine |
what are the precautions and side effects of Pralidoxime (2PAM)
Tachycardia
hypertension dizziness |
What is the dosage for Pralidoxime (2PAM)
1-2G in 250-500 ML NS unfused over 15-30 minutes; if no iv access is available then 1-2g IM/SC
What is the drug class for Sodium Bicarbonate
Alkalinizing Agent
Pharmacological actions of Sodium Bicarbonate
Raises the pH of blood plasma by buffering excess hydrogen ionds. Provides vascular bicarbonate to assist in the buffer system in reducing the effects of metabolic acidosis and in the treatments of some overdoses.
What are the indications for Sodium Bicarbonate
-Prolonged cardiac arrest
- Known hyperlkalemia -Tryciclic antidepressant t/barbiturates - Refractory metabolic acidosis |
what are the contraindications for Sodium Bicarbonate.
None in the emergency settings
What are the precautions and side effects for Sodium Bicarbonate
-Extracellular alkalosis
-Cardiac Arrhythmias -Decreased cerebral and cardiac functioning |
What are the dosages for Sodium Bicarbonate
1meg/kg IV/IO may repeat at half dose ever 10 minutes
What is the class for Verapamil
Calcium channel blocker
What is the pharmacological actions of Verapamil
Slows the AV conduction and suppresses reentry dysrhythmias. Causes dilation of the coronary arteries and reduces the oxygenate demands of the heart
What are the indications of Verapamil
SVT refractory to adenosine
Atrial Flutter Atrial Fibrillation |
What are the contraindications for Verapamil
-hypotension
- Cardiogenic shock - Heart block - Congestive heart failure - WPW syndrome with atrial fibrillation |
What are the precautions and side effects for Verapamil
hypotension
Heart block CHF Headache Dizziness |
What is the dosage for Verapamil
2.5-5.0 MG pushed over 2-3 minutes may repeat at double dose every 15-30 minutes to a max total dose of 20MG
