Vasospastic Angina
vasospasm of coranary arteries
Vasospatic angina drugs
nitrates and CCB (both)
B-Blocker clinical use
Post MI, Stable angina, prevent stroke, HTN
First line therapy for stable angina
B-blocker
B-blocker contraindications
Asthma, unstable heart failure, pheocromocytoma, av block, pulmonary edma
Nitrates clinical use
Both stable and vasospastic angina
Both acutely and prophylacticly
Nitrates MOA
Stable (decreases O2 demand via decrease in PVR)
Vasospastic (increases O2 supply via dilation of coranary arteries)
Nitrate Side effects
headache, flushing, orthostatic hypotension, dizziness, reflex tachycardia
Nitroglycerin + B-blocker
prevent reflex tachy
DHP CCB
decreases afterload, get reflex tachy
Non-DHP CCB
Decreases afterload, HR, Contractility and ultimiatley O2 demand
CCB
Dilate epicardial coronary arteries, increases O2 supply
Ranolazine
Decrease late Na influx which decreases Ca overload, no effect on HR or BP, for stable angina
Ranolazine side effect
dizziness, headache, constipation, nausea, Prolongs QT (causes bad arythmias)
Prophylatic ischemic drugs
B-blocker, CCB (both), Nitrates (both), Ranolazine
Actue ischemic drugs
Nitrates (both)
