Class

Sympathomimetic; vasopressor

Mech

Dopamine stimulates alpha beta1 and dopaminergic receptors. Acts indirectly by causing release of norepi from storage sites in sympathetic nerve endings. The effects of dopamine are dose related. At low doses b1 and dopaminergic receptors are simulated. At high doses alpha effects predominate

Onset

IV drip has rapid onset and lasts as long as drug is being infused

Indications

Symptomatic hypotension. With exception of cardiogenic shock, fluid resuscitation must be attempted before dopamine



Symptomatic bradycardia refractory to atropine

Contra

Shock in the setting of trauma



Uncorrectd tachydysrhythmias except sinus tach



Pheochromocytoma

Precautions

Extravasation causes tissue necrosis

Side effects

Increased myocardial demand. Ischemia. Tachydysrhythmias

Interactions

Sodium bicarb can deactivate catecholamines. MAO inhibitors potentiate the effects

Dosage

5-20 mcg/kg/minute IV infusion



Improve cardiac output (beta) 5-10mcg/kg/min



Increase peripheral basically resistance (alpha) 10/20mcg/kg/min

How supplied

Prefilled syringes and ampules of either 400 or 800mg. Premix bags with either 400 or 800 mg in 250-500ml. Concentrations of mixtures vary but most commonly are 1600 mcg/ml
