airway abnormality characterized by mechanical impediment of delivery or O2 in lungs
Airway Obstruction
How many children die each year from foreign body aspiration in US
500
What percentage of children die from foreign aspirations and what age does this occur in children
90 percent
children younger than 3 years
65 percent daeths occur with this age group
infants
Objects in denistry are at great risk to fall into the
posterior portion of the oral cavity and into the pharnyx
what are some dental hazards
aspirations of objects
ingestion of objects
what are some hazards of ingesting an object?
10% cause impaction, abscesses, or perforation of the GI tract; 90% pass through tract in 10-12 hours
What are the hazards of aspirating an object?
Extremely serious; Must be removed to avoid major infection or death
What are some dental devices recovered from the throat?
crowns
endo instruments
mirror heads
rubber dam clamps
mouth and nose empty into
pharynx
The first passage of airway is
trachea
aka windpipe
the trachea is located
in front of throat and carries air from pharynx to the lungs
The second passage and what does it carry
the esophagus solids from the mouth to the stomach
What are two classifications of airway obstructions
Mild airway (partial obstruction)Severe obstruction (complete)
what is a mild airway obstruction
good air exchange;
responsive and can cough forcefully;
wheeze between coughs
severe airway obstruction
poor or no air; weak cough or no cough; high pitched noise while inhailing or no noise;increased respiratory difficulty; unable speak; grasping neck; unable to move air
what is the first thing you do if pt is conscious with partial obstructed airway
ask "Are you choking" ;as long as good air exchange stay with pt and monitor coughing and breathing. If persist activate EMT
How do you treat a pt with complete airway obstruction
Ask victim if chocking; If pt nodes YES and no speech then activate EMS; Perform Heimlich maneuver until object expelled or pt becomes passes out
how do you treat unresponsive choking victim
activate EMS; open airway; remove object if seen; CPR;
how do treat a unresponsive child choking victim
if alone with child, open airway; remove object if seen; CPR : 5 cycles or 2 minutes CPR then activate EMS
Unresponsive victim you need to do this everytime you u give CPR to unresponsive victim
Look for object and if u see it remove
How can you tell if treatment in unresponsive pt is a success
see chest rise when giving breathes; See and remove object
After relieving choking to unresponsive pt
provide 2 breathes: check pulse
unresponsive pt with nopulse/no breathing
chest compressions and AED
unresponsive pt with pulse but no breathing
CPR 1 breathe every 5 to 6 sec (10 to 12/min and check pulse every 2 min)
unresponsive choking victim with pulse and breathing
place pt in the recovery position (laying on the side) and monitor until EMT arrive
Where do u place fist during heimlich
place thumb side of the fist agaisnt the pt abdomen; thenis held midline slightly above the umbilicus and below xyphoid
Prior to dismiss of pt choking it is suggested
pt be evaluated for possible complications by medical
If pt is pregnant and choking wher4e do you place fist
perform chest thrusts instead of abdomen
Define hyperventilation and when does it occur
increase in the rate or depth of breathing that result in a change in chemistry; occurs as a result of anxiety
Hyperventilation results in
alkalosis
predisposing factors of hyperventilation
acute anxiety; hidden fear
signs and symptoms hyperventilation
nervousness; increase rate respirations; feeling suffocation
tightness chest; dizziness; tingling in extremities
what do you do if pt is hyperventillating
stop procedure; position pt to comfort; remove materials from mouth; calm pt; CPR/ breathe into hands; find cause, discharge
extreme case of hyperventilation
administer drug therapy/ Diazepam;
How to prevent hyperventilation
Medical history quetionare; modify dental treatment to lesson fear; physical exam; vital signs
define asthma
affliction of the respiratory tract that can affect all aspects of the tracheobranchical tree
tracheobronchical tree
trachea; bronchi; bronchioles
types of asthma
extrinsic; intrinsic; status asthmaticus
Etiologic factors in acute asthma
allergy; respiratory infection; physical exertion; occupational stimuli; pharmacologic stimuli; psychologic factors
common allergens in dental office
eugenol; impression material; resins; latex
signs and syptoms of dental allergens
coughing; sweating; tightness in chest; difficulty breathing; wheezing; BP normal/elevated increase heart rate; nervousness
If symptoms start what do u do
stop dental tx/ evaluate respiration
If pt has signs/symptoms of allergies, what do you do?
1.stop tx; 2. position pt; 3.ABC/CPR 4.remove allergen 5.administer-bronchodialator-O2-epinephrine IV- 911
Symptoms of cardio-related hyperventillation?
Palpations ;Tachycardia(rapid hr) ; Precordial pain(chest pain)
Neurologic symptoms of hyperventilation?
Dizziness; lightheadedness;
Identify one sign of hyperventilation?
nervousness of feeling suffocation
Treatment for hyperventilation?
remove precipitating cause;
postion pt upright; Instruct pt in proper breathing;
What is proper breathing technique to show pt when they are hyperventilating?
slow deep breathing and to cover nose/mouth with hands. Or can use oxygen tank mask
Why do we hav the pt cover there nose and mouth and breathe?
This is done to correct respiratory alkalosis.
Identify what vital signs need to be monitored? normal range
Adult Pulse 60-80
Pediatric Pulse 60-140
Adult Respirations 12-20
Pediatric 15-25
What are two signs of respiratory airway obstruction?
Inability to speak, breathe, cough; Pt demonstrates Universal sign for choking
Two signs of partial airway obstruction?
Wheezing between coughs; forceful cough; weak speech
What do you do if your pt is choking and you can see the object?
Position pt in Trandelenberg(on side) use forceps or suction if the object
What do you do if pt is choking but cant see object?
Administer heimlich maneuver
(perfomed from behind w/pt in an upright position in the dental chair
