Poxviridae
Family of Class I DNA viruses. Enveloped. Largest viruses after mimivirus. Cause dermatic pustules (pox).
Poxviridae Members
Smallpox, cowpox, molluscum contagiosum. NOT syphilis or chickenpox.
Chordopoxvirinae
Subfamily of organisms with back bones. Genus Molluscipoxvirus and Genus Orthopoxvirus cause disease in humans.
Entomopoxvirinae
3 genera of insect viruses.
Molluscum Contagiosum
Prevalent, 4 serotypes. Transmission thru contact or fomites. Symptomatic papules. MCV-1 face, trunk, limbs in children via team sports. MCV-2 midsection in adults via sex.
MCV & Wart Treatment
Often self-resolving. At home: wart remover, Retin-A, duct tape (impractical, 6 weeks), cotton ball soaked in apple cider vinegar (24 hrs). Clinical: liquid nitrogen, laser, Cantharadin (blistering agent).
Smallpox Transmission
Only in humans. Physical or close contact necessary: infected saliva expelled & respiratory particles inhaled by noninfected.
Smallpox Symptoms
Fever and malaise. Rash from head down: Day 3 macule, Day 4-5 papule, Day 7 vesicle, Day 9 pustule, Day 13 scab.
Smallpox Treatment
Isolation. Vaccine 1-4 days after exposure helps. "Off label" Cidofovir - not intended to treat smallpox. Vaccinia antibodies (passive immunization).
Smallpox Vaccine
Dryvax (live vaccinia virus). 95% effective. Bifurcated needle punctures skin and reaction should develop.
Verruca Warts
Class I. Nonenveloped icosahedral. Caused by papillomaviridae or polyomaviridae.
Wart Transmission
Autoinnoculation, direct, or indirect.
Wart Symptoms
Common (high-contact areas), filiform (eyelid, armpit, neck), genital, flat (face, hand, wrist), plantar (foot).
Cancerous Warts
HPV-16 and HPV-18 (possibly -3, -6, and -11) cause cervical cancer. May cause penile cancer.
Wart Prevention & Control
Gardasil vaccine: synthetic structured vaccine (capsomer units) given in 3 doses.
Hepatitis
Inflammation of the liver: destroys hepatocytes, infiltrates immune system, causes swelling resulting in dysfunction. Jaundice - yellowing of eyes and skin.
Hepatitis A
Picornaviridae, Class IV. Fecal-oral, direct or indirect transmission. Ig or fluid treatment.
Hep A Vaccines
Havrix-R and Vaqtar inactivated, use aluminum hydroxide as an adjuvant.
Hepatitis B
Hepadnaviridae, Class VII. Blood, sex, congenital transmission. Ig, IFN, nucleoside analogue treatments.
Hep B Vaccines
Engerix-B, Recombivax, and H-B Vax II recombinant HBsAg surface antigen in yeast cells. Heptavax-B plasma-derived foreign use.
Hepatitis C
Flaviviridae, Class IV. Blood or congenital transmission. IFN & ribavirin treatments.
Hepatitis D
Delta agent, requires HBsAg. Satellite nucleic acid, Class V. Transmitted w/ Hep B. Same treatment as Hep B.
Hepatitis E
Caliciviridae, Class IV. Fecal-oral, direct or indirect transmission. No treatment.
Hepatitis G
Flaviviridae, Class IV. Blood transmission. No treatment or prevention.
IFN Treatment
Produced by leukocytes and fibroblasts. Antiviral and anticancer. Pegylated and recombinant in hepatitides.
