Indications

Acute Cardiogenic Pulmonary Edema secondary to CHF, Cerebral edema secondary to Traumatic Brain Injury (TBI), High Altitude Pulmonary Edema (HAPE), High Atitude Cerebral Edema (HACE), Toxic exposures to medications/poisons, Hyperkalemia, Rhabdomyolysis

Mechanism of Action (Furosemide)

Loop diuretic with rapid-acting and antihypertensive effects that inhibits sodium re-absorption by the kidney, Vasodilation effect reduces venous return and cardiac preload (workload)

Mechanism of Action (Bumetanide)

Loop diuretic with 40x greater potency than furosemide, shorter duration and may produce mild hypotension

Mechanism of Action (Mannitol)

Osmotic diuretic that draws water into the intravascular space thorugh its hypertonic effects, causes diuresis

Adult: 40-120 mg slow IVP


Pedi: 1 mg/kg slow IVP

Side Effects

Hypotension, hypovolemia, (w/ significant fluid and electrolyte depletion rates), hepatic coma

Contraindications

Do not use in physiologically unstable patients unable to produce urine such as CRF (chronic renal failure) patients

Precautions

Geriatric patients due to possible hepatic impairment or nephritic syndrome, patients receiving digitalis or potassium-depleting medications
