Q. 9 DDx for chest pain
Angina Pectoris,
Dissecting Aortic Aneurysm,
Gastroesophageal Reflux,

Q. Angina Pectoris
Tightness, pressure, pain in chest
Usu felt retrosternally
Usu lasts 2 - 10 min
May radiate to neck, jaw, shoulder, arms
<Exertion, stress
>Rest
REFER TO ED!!
Q. Dissecting Aortic Aneurysm
ER!
Excruciating, tearing pain, Sudden onset
Not relieved by rest, Not worse w/breathing
May radiate to back or abdomen
May have palpable abdominal mass/tenderness
Q. Gastroesophageal Reflux
Retrosternal burning sensation
Can radiate upward
Acidic, salty, or bitter fluid in mouth
< supine, bending forward, or post large meal
Chronic cough, chronic laryngitis
Q. Costochondritis
Pain at costochondral junctions.
Usually elicited by palpating the inflamed joint
Hx minor trauma or unaccustomed activity (ie painting, moving furniture)
Q. Lung Abcess
Cough w/purulent, foul smelling, blood tinged sputum
Affected area dull to percussion
Decrease breath sounds, may hear Crackles
Fever, chills, anorexia, night sweats
Q. Acute MI
ER!
Sudden Substernal Pain
Not relieved by rest or nitroglycerine
May radiate to L arm, jaw, neck, shoulder
Painful breathing
Anxiety, sense of impending doom
N/V, Fatigue, indigestion, SOB
Hx hyperlipidemia or DM
Q. Pericarditis
ER!!
Sharp, stabbing, knife-like chest pain
May radiate to back, neck, shoulder, arm
<Deep inspiration, lying supine
>Sitting up, leaning forward
Friction rub on auscultation
Fever, dyspnea, tachycardia
Q. Pneumonia
Painful breathing
Pain Increase w/deep breathing
Shaking chills, fever
Dry cough that becomes productive
Blood-tinged, green, foul-smelling sputum
Crackles, rhonchi
Q. Pneumothorax

Severe, sudden, sharp, diffuse/localized chest pain

Usu unilateral
