Film-based Workflow
Process Order:
Image acquisition - Processing of film - Film Hang and Review - Send to the Film Room for storage - Available from the film room for education, critical care, and physician's offices |
Digital-Based PACS Workflow
Process Order:
Image acquisition - Image is immediately available on PACS Server - Radiologist or other specialists can review and it goes to Archive - Available from archive anytime on a PACS workstation anywhere via electronic medical record. |
HIS
Hospital Information System:
Designed for tracking patient admission information, clinical evaluation, treatment, discharge information, and billing. It is for demographics - global information that everyone will need. |
RIS
Radiology Information System:
Designed to track imaging history and scheduling information. Is department specific information for only those who need it. |
HIS and RIS Working Together
HIS and RIS work together to combine information for a total patient file. For instance, a Rad can pull up a file for a patient and get all their demographic information from HIS which will match to the imaging files from the RIS.

HL7
Health Level Seven is a set of protocols that sets the standard for exchanging information between medical applications. It defines the format and content of messages that must be used to exchange data. Works with DICOM standards.
HL7 Purpose
Standardizes information for data exchange between RIS/HIS, LAN/WAN, On-site/Off-site, etc...
Newest HL7
XML - eXtensible Markup Language
- organizes test information with labels - is a data format for sharing data so that syntax is tight - Cleans up the pipelines where no data is entered (so it is not just a bit stream of junk!) - Helps meet integration needs for data |
IHE initiative
Integrating the Healthcare Enterprise Initiative:
Is the vision for the future where there are electronic health records. The idea is that we would have our own personal electronic health records at home that we can transfer to hospitals, clinics, etc... creating a national health records system. |
3 Ways of Integrating Information in Your System
Worst Way - Double entry due to NO communication between RIS/HIS.
Next Worst Way - Buy one overall package for hospital, but since there is no specialization for each system part, it will do everything, but NOT WELL! Ideal Way - Buy individual programs so each is specialized and works well, but make them all work together (integrate) with others. So, buy separate PACS, RIS, HIS systems but don't pay for them until everything is working together. |
