What is the Sandovsky method?
done after 24 wks
eat small meal then lie down on L side for 1 hr and count how many times fetus moves
should be about 10 times
call Dr. if 7 or less
What is the Cardiff method?
count fetus movement until 10 and see how long it takes
call Dr. if more than 1 hr
What should the fetal heart rate be?
120-160
What is the rhythm strip testing?
look at fetal heart rate for 20 min
should have 2 accelerations of the heart that rise about baseline 15 beats for at least 15 sec
What is short term variability?
how the heart rate changes from beat to beat
What is long term variability?
how the heart rate changes over time
What is a reactive NST?
2 accelerations of 15 beats remaining at that level for 15 seconds in a 10 minute period
What is a nonreactive NST?
none or <2 accelerations
needs retesting
What is an OCT?
decreases 02 to the fetus to see reaction
done on a term baby
get baseline HR
increase ocytocin until 3 contractions in 10 min that last at least 40 sec
usually done after nonreactive NST
What is contraindicated for an OCT?
bleeding
incompetent cervix
pre-term
What is a negative OCT?
no decelerations present with contractions
What is a positive OCT?
50% or more of contractions are accompained by late deceleration
needs further evaluation
What is a suspicious OCT?
1 late or difficult to read
What is a BPD?
measurement of head to determine growth
if about 8.5 cm the fetus should be about 5.5 lbs and 38 weeks
What should the head circumference be at about 38-40 wks?
34.5 cm
What is placental grading?
determines wks by looking at calcium deposits
0 = 12-24 wks
1= 30-32 wks
2 = 36 wks
3 = 38 wks maturity
What is doppler umbilical velocimetry?
looks at flow of blood through placenta to umbilical cord to see if there is any resistnace
What would a decreased doppler flow indicate?
poor neonatal outcome
immediate intervention
What is an amniotic fluid volume assessment?
measures amniotic fluid
What should the anminotic fluid be at 20 wks or less?
2 pockets of fluid
What is a negative OCT?
no decelerations present with contractions
What is a positive OCT?
50% or more of contractions are accompained by late deceleration
needs further evaluation
What is a suspicious OCT?
1 late or difficult to read

What would a decreased doppler flow indicate?

immediate intervention
What is an amniotic fluid volume assessment?
measures amniotic fluid
What should the anminotic fluid be at 28 wks?
15 cm
What is a negative OCT?
no decelerations present with contractions
What is a positive OCT?
50% or more of contractions are accompained by late deceleration
needs further evaluation
What is a suspicious OCT?

What is an amniotic fluid volume assessment?
measures amniotic fluid
What should the anminotic fluid be at 28-40 wks?
15 cm
What would a measurement of 20-24 cm of amniotic fluid indicate?
Polyhydramnios
What would a measurement of 5-6 cm of amniotic fluid indicate?
Oligohydramnios
What is the BPP?
fetal breathing movements
fetal movement
fetal tone
amniotic fluid volume
placental grading
fetal heart rate reactivity
What is a good BPP?
6-8
What BPP score indicates fetal jeopardy?
4-6
What's McDonalds rule?
measures the distance from the fundus to the synthesis pubis in cm
@ 20 wks should be at the level of mom's umbilicalus
20-31 cm = wks gestation
What would an increased in McDonalds rule indicate?
Polyhydramnios or multiples
What would a decrease in McDonalds rule indicate?
Oligohydramnios or uterine growth retardation
What is the alpha fetoproein?
most accurate at predicting neural tube defects
What is the triple screen?
more accurate predictor for down syndrome
What is chrionic villi sampling?
may be done if a family hx of downs or other ds and may terminate pregnancy
What does the mother have to be treated with if she's rh-?
rogan
