What kind of depression is it used for?
Desipramine, used for MAJOR and SEVERE depression.
How does Desipramine work?

What does it reduce?
Blocks the uptake of NE AND serotonin into nerve terminals. Allows neurotransmitters to accumulate at synapse, leading to prolonged stimulation of receptors and eventual desensitization.

Reduces the morbid preoccupation in 50-70% of pts.
Antidepressants do not cause what?
A stimulant or "high".
What is the onset of action for Desipramine?
About 2 weeks, but 6-12 weeks for the full effect.
CVS SE of Despiramine?

CNS SE?
Cardiac arrhythmias, orthostatic hypotension and reflex tachycardia.

Nardil. Dinosaur of anti-depressants. Inhibits monoamine oxidase, enzyme that breaks down NE, DA and serotonin, so more of these accumulated at synapse.
What is a contraindication of using MOI's?
Pts using SSRI's and other anti-depressants should not be treated with MOIs
What is lithium used for?
To treat mania.
What is important to know about using lithium?
Has a narrow therapeutic index!! Mania pts tend to be forgetful and may forget to take meds. Can be very dangerous if they try to catch up and take several pills at once.
SE of Lithium?
Ataxia, tremors, confusion, convulsions.
