Indications

To reduce inflammation, pain & edema; facilitates movement & reduces spasticity. REDUCES edema, does not remove it.

Contraindications

-hypersensitivity

-intolerance

-Raynaud's Syndrome

-regenerating peripheral nerve

-cryoglobulinemia (serum protein in distal circulation impairs flow)

-PVD

Physiological Effects - Hemodynamics

-Vasoconstriction - reduce blood flow -prevents more blood from being cooled

-Decreased blood flow - decreased temp results in increased viscosity

-Vasodilation - Hunting response (tissue becomes cold, body responds with cold-induced vasodilation

-Tissue temp change - decrease in temp to 10C or more may cause frostbite

Neuromuscular Effects

-Change in viscoelastic properties of tissue - tissue is cooled may not respond favorable to length change

-Decreased nerve conduction velocity - 20-30 min to

-Increased pain threshold

-Altered muscle strength following cold (perform MMT first)

Metabolic Effects

-chemical reactions slowed, slows inflammatory response

-can reduce collagen destruction in inflammatory joint disease (RA)

Depth of cooling

1-4 cm

Cold/Ice pack

Conduction

10-15 minutes

advantage - different sizes and shapes, reusable, safe while unattened

Ice Massage

Conduction

-directly on skin

apply to small area

time - until numb

CBAAN - cold, burning, aching, analgesia, numb

advantage - quick effect for friction message

Cold compression/Cryocuff

Conduction

20 minutes

indications - pain, edema, acute inflammation

adverse effects - frostbite, tissue death, nerve damage, unwanted vasodialation

cold and compression together = less pain and greater ROM

Cold whirlpool/cold bath

Convection

advantage - effective for distal extremities, total contact of area, home use

disadvantage - extremity must be in a gravity dependent position

55-64 degrees

15-20 minutes

Contrast Bath

-distal extremity

-alternate cold/hot

-edema reduction (start hot, end cold)

-pain control (start and end hot)

-indications: athletic injuries, leg ulcer, ortho problems, arthritis

-contraindications: cardiovascular problems, PVD, sensation, tend to hemorrhage, pregnacy

-Hot: 4-5 minutes (100-110 F)

-Cold: 1 minute (55-65 F)

Normal Response

-decreased edema, temp, metabolic rate, tone, nerve conduction velocity

-increased pain threshold

Abnormal Response

rash, severe pain, numbness, blood in urine

Common Conditions

Acute injury or trauma, orthopedic surgery, after exercise for DOMS, joint or soft tissue mobs, osteoarthritis and rheumatoid arthritis (chronic inflammation), pain-spasm-pain cycle, spasticity due to UMN dysfunction, flaccidity (quick icing - but unreliable) from UMN, reduce sensation of pain prior to athletic strengthening & stretching
