CN I

(Olfactory)


Ask your patient to identify at least two common substances such as coffee and cinnamon.


*Make sure nostrils are patent before performing this test

CN II

(Optic)


Test visual acuity with a Snellen chart and the Rosenbaum near-vision card.

CN III, IV, and VI

(Oculomotor, Trochlear, and Abducens)


Assess these nerves together using the corneal light reflex test, the six cardinal positions of gaze, and the cover-uncover test. Also inspect the size, shape, and symmetry of your patient's pupils and papillary reaction to light.

CN V

(Trigeminal)


Sensory- Ask your patient to close his eyes and then touch him with a wisp of cotton on his forehead, cheek, and jaw on each side.


Pain Perception- touch the tip of a safety pin to the same three areas. Ask patient to describe and compare both sensations.


Motor- Ask patient to clench teeth while you palpate the temporal and master muscles (should be equal bilaterally)


*If patient isn't alert test corneal reflex but lightly touching eye with a wisp of cotton.

CN VII

(Facial)


Sensory- test taste by placing items on anterior portion of patient's tongue (sweet, sour, and bitter)


Motor- observe facial symmetry at rest while patient smiles, frowns, and raises eyebrows


Muscle Strength- have patient close eyes tightly and attempt to open their eyes

CN VII

(Acoustic)


Weber's test- strike a tuning fork and place it against patient's forehead or top of head


Rinne test- strike a tuning fork and place over mastoid process

CN IX and X

(Glossopharyngeal and Vagus)


Listen to patient's voice


Check gag reflex by touching the tip of a tongue blade against his posterior pharynx and asking him to open wide and say "ah!"


Watch for symmetrical upward movement of the soft palate and uvula and for the midline position of the uvula

CN XI

(Spinal Accessory)


Test the strength of the sternocleidomastoid muscle and the upper portion of the trapezius muscle

CN XII

(Hypoglossal)


Observe your patient's tongue for symmetry (should be midline without tremors or muscle twitching)


Test tongue strength by asking patient to push his tongue against his cheek as you apply resistance
