Gather equiptment: Secondary IVF bag; IV tubing secondary kit; needless syringes; blunt cannulas; alcohol pads; medication labels; medication; medication to be piggy backed; a needle; and a syringe for medication.

Perform Hand Hygiene; Don Clean Gloves; open supplies without contaminating.

Check medication again; draw up calculated medication with needless system; label syringe; Place medication into the piggyback through medication port and gently mix the solution; label the bag with medication added (orange).

Close roller clamp on tubing; Open the other port (larger opening) and spike the piggy back with the secondary tubing (do not contaminate); be sure to have leverlock cannula in place (aligator clamp); hang bag on IV pole and allow tubing to prime; ensure air is removed; allow solution to flow very slowly due to little amt available in bag; close roller clamp; recheck all is labeled.

Assess IV site for signs of phlebitis or infiltration; Assess IV site for complications: REEDA, redness, warmth, drainage, pain, and swelling.

Hang piggy back bag on the IV at the same level as the maintenance bag for now; indetify the highest needless IV port in the maintenance line; clean the port with an alcohol wipe, and allow to dry.

Remove the tip cap of the leverlock cannula; (DO NOT CONTAMINATE); Connect the piggy back to the maintenance line; lower the IV bag using the (blue) hook to allow the piggy back medication to flow first; open the roller clamp; and set drip rate according to orders.

Re-position pt; provide for safety; remove gloves; perform hand hygiene; document the procedure in MAR (time, route, initials, condition of IV site prior to medication administration, pt. tolerance; and periodically reassess IV site and infusion rate at pt. bedside; return to d/c piggy back at correct time on orders.

Gather equipt; enter pt.'s room; intro self; verify pt. use two identifiers; verify allergies; verify what medication is for; verify any questions; provide for privacy; perform hand hygiene, and don clean gloves.
