Not autoimmume
Not systemtic
"wear and tear"
Joints lose cartilage and rub on each other
May have bone spurs
Pathophysiology of Osteoarthritis
Early stage of Osteoarthritis
joint pain and stiffness that is relieved with rest
Late stage of Osteoarthritis
pain with minimal motion and even at rest
Types of Analgesics
Tylenol
ASA
NSAIDS (cox1 and cox2)
Cortizone injection
Most common joint replacement surgery
May last up to 20 years
Last resort
Total hip
Chronic, progressive systematic, inflammatory autoimmune disease
Usually feel better with activity
Normally bilarteral and symmetric (equal on both sides)
Rheumatoid Arthritis
Often associated with RA
Sjogrens Syndrome
Feltys Syndrome
Caplan Syndrome
Causes dry eyes, mouth and vagina
- then causes vision problems and cavities
Sjogrens Syndrome
Leucopenia, enlarged liver and spleen
- decreased WBC
- Immunocompromised
Feltys Syndrome
Nodules in the lungs
Caplan Syndrome
0-39 is normal
- measures for unusual antibodies of immunoglobin (g) and (m) IgG, IgM
RF - Rheumatoid Factor
Measures the titer of unual antibodies that can destroy cells
- is a value higher than 1:8
ANA or FANA - Antinuclear Antibody Titer
Looks for inflammation or infection
- 0-20 is ok
- 20-40 mild
- 40-70 moderate
- 70-150 severe
ESR - Erthrocyte Sedimentation Rate or Sed Rate
0.8 to 3.1 is normal
Measureas inflammation
CRP - C-Reactive Protein
Ibuprofen
Celebrex
Salicylates (aspirin)
NSAIDS
Common side effect of NSAIDS
Tinnitis- ringing in the ears
Plagueril
Sulfasalazine
Minocycline
Disease Modifying Antiheumatic Drugs (DMARDS)
Takes 4 to 6 weeks to work
side effects include decreased WBC and platelets and increased enzymes and creatine
NO ALCOHOL
Causes major birth defects
Methotrexate
Bilogical Respnse Modifier
- stimulates or restores the ability of the immune system to fight arthritis
- Given as single IV infusion every 2 to 8 weeks (using the lease amount possible)
- Can cause a sever reaction
Remicade
Prednisone
Medrol
- may cause: weight gain, decreased immune system, edema, insomnia, decreased healing, increased glucose level, *moon face*, think skin, and *decreased bone density*
- no good for long term treatment
Glucocorticoids or steroids
Classification of Lupus
- Effects the only the skin
- will have discoid (coinlike) leasions or scarring evident on the skin - especiall when exposed to sunlight, vutterfly rash, and alopecia
DLE- Discoid Lupus Erythematosus
Classification of Lupus
- Has systemic effects
- Chronic and progressive
- Causes major body organs and systems to fail
- Can have periods of remissions and exacerbations (flare-ups)
- Kidney failure is the leading cause of death
- Likes to pick on one organ
- Will have decreased urine output, increased BUN and creating, and will be SOB
SLE - Systemic Lupus Erythematosus
- Hardening of the skin
- Chronic connective tissue disease
- Patients do not respond well to steroids or immunosuppressants
- Will have a very tight face from lose of elasticity in the connective tissue
- Skill will be taut, skiny, and clear of wrinkles - then the skin hardens and thickens
- may effect GI, cardiac, pulmonary, and renal systems
Progressive Systemic Scleroderma (PSS)
Systemic disease which urate crystals deposit in the joints causing inflammation
Meds used:
-NSAIDS and Colchicine for acute
- Allopurinol and Probenecid for chronic
- Aspirin and diurectis should be avoided
Diet: limit soda and drink lots of water
Gout
Most common form of gout
- results from one of several errors of metabolism - causing an end product of uric acid excreted to the kidneys
- When the uric acid excededs the bodies ability to secrete it, it is deposited in the synovium (joints)
Primary Gout
Chronic pain syndrome
Not inflammatory disease
Has specific "trigger points"
Can be very sensitive to pain
May also have Chronic Fatigue Syndrome and often become depressed and anxious
No diagnostic test
Fribromyalgia (FMS)
