what is thyroiglossal duct
a tubular structure through which the thyroid gland migrates...

begins at foramen cecum (at tongue basE) , extends anterior to the thyrohyoid membrane and strap muscles, and ends at level of thyroid isthmus
when does thyroglossal duct normally involute
8-10 weeks gestation
pathophys of thryoglossal duct cyst
if any portion of the thryoglossal duct fails to involute, a cyst or sinus tract can form (b/c duct is lined by secretory epithelium)
are the majority of thyroglossal duct cysts midline or off midline
75% are midline,
most are at or below hyoid bone
most common midline neck mass
thyroglossal duct cyst
appearance of thyroglossal duct cyst
cystic mass with thin peripheral rim of capsular enhancement
pathophys of laryngocele
develops from increased laryngeal pressure (glass blowers, musicians)
abnormal dilatation of ht appendix of the laryngeal ventricle (separating the false and true cords)
describe location of laryngocele
they are both in and outside of thyrohyoid membrane most commonly (although they can be either above or below)

usually present as a lateral neck mass near hyoid bone
what should the formation of a laryngocele in absence of risk factors suggest
a neoplasm obstructing the laryngeal ventricle
which branchial cleft do most of the branchial cleft cysts arise from
2nd
how many branchial clefts are there
6
course of 2nd branchial cleft
begins at base of tonsillar fossa dn extends between internal and external carotid arteries
most common location of a 2nd branchial cleft cyst
usually anterior to the middle portion fSCM
lateral to internal jugular vein at level of carotid bifurcation
types of lymphangiomas
capiilary
cavernous
cystic
how does the lymphatic system develop
how does lymphangioma develop
from primitive embryonic lymph sacs that are derived from venous system

if the lymph sacs fail to communicate with venous system, then lymph sacs dilate with lymph --> lymphangioma

if a single region fails to communicate--> cystic hygroma
extensive lack of communication --> fetal hydrops
medical conditions assoc with cystic hygroma
turner syndrome
fetal alcohol syndrome
noonan sydrome
