Dental plaque induced gingivitis
-adolescents and adults
-gingival redness, bleeding and enlargement, absence of cal and bone loss
-inadequate oral hygiene
-no specific bacteria, high levels of streptococcus sanguis,s. mitis, fusobacterium nucleatum, actinomyces viscosus, and veillonella parvula have been detected
Drug influenced gingivitis
-patients taking phenytoin, calcium channel blockers, cyclosporine
-gingival enlargement or overgrowth, absence of cal and bone loss
-medications and dental plaque
-no specific bacteria
Gingival diseases associated with the endocrine system
-pregnant pts and adolescents
-intense gingival inflammation, absence of cal and bone loss
-changes in sex hormone level, exaggerated response to plaque
-prevotella intermedia
Gingival manifestations of systemic conditions, mucocutaneous disorders
-these include skin disease including pemphigus, lichen planus, beign mucous membrane pemphigoid(BMMP), and others
-gingiva may appear blotchy red, have blisters or vesicles, or slough off from the underlying connective tissue
-abnorality in the immune system
Chronic periodontitis
-most prevalent in adults but can occur in children and adolescents
-pocket information,bone loss,inflammation
-primary etiologic factor is dental plaque, many other like smoking and systemic diseases
-high numbers of porphyromonas gingivalis, bacteroides forsythus, prevotella intermedia and eikenella corrodens
Generalized aggressive periodontitis
-usually affects persons under 30 years of age
-generalized: severe gingival inflammation rapid bone destruction affecting at least three permanent teeth other than the first molars and incisors
-defect in polymorphonnuclear (PMN) and/or macrophage function
-some predominant bacteria include prevotella intermedia, actinobacilus, actinomycetemcomitans
Localized aggressive periodontitis
-except for the presence of periodontitis patients are otherwise clinically healthy, circumpubital onset
-sparse amount of plaque and calculus, little inflammation..localized: incisor and first molar sites
-defect in PMN chemotaxis
-actinobacillus,actinomycetemcomitans,capnocytophaga species
Necrotizing periodontal diseases (NUG)
-Young adults
-punched out interdental papillae, pain, and free bleeding
-inadequate oral hygiene,stress,cigarette smoking
-specific bacteria:prevotella intermedia,fusobacterium nucleatum
Necrotizing periodontal diseases (NUG)
-HIV infected individuals
-distinct red band around marginal gingiva, periodontal involvement of deeper sturtures, rapid bone loss and spontaneous gingival bleeding
-immunodeficiency(decrease in CD4 + lymphocytes)
-similar to chronic periodontitis
-bacteria:porphyromonas gingivalis,prevotella intermedia,f. nucleatum, candida albicans (fungus)
Refactory periodontitis
-can occur in any case of perio disease
-continued periodontal destruction despite appropriate treatment
-dental plaque is the cause
-numberous bacteria, high levels of bacteroides forsythus, porphyromonas gingivalis
