Clotting Factors
Platelets
Fibrinogen
Prothrombin
Factors V and VII
Idiopathic Thrombocytopenic Purpura (ITP)
Autoimmune disorder in which antiplatelet antibodies decrease the life span of the platelets
ITP
Diagnostic finding:
Thrombocytopenia
Capillary fragility
Increased bleeding time
ITP
Complications:
Severe hemorrhage after cesarean birth or from cervical or vaginal lacerations

Increased risk for postpartum uterine bleeding and vaginal hematomas
ITP
Treatment:
Corticosteroids or IV immune globulin
Platelet transfusions
Splenectomy if unresponsive to medical treatment
Von Willebrand Disease (VWD)
Form of hemophilia
factor VIII deficiency and platelet dysfunction
Most common hereditary bleeding disorder
VWD
Sign/Symptom
Familial bleeding tendency
Previous bleeding episodes
Prolonged bleeding time (most important)
factor VIII deficiency
Bleeding from mucous membranes
VWD
Risk for bleeding after birth
Up to 4 weeks
VWD
Treatment
desmopressin IV, PO or nasally (promotes release of vW factor and factor VIII
Transfusion with plasma containing vW factor and factor VIII
Concentrates of antihemophiliac factor (Humate-P, Alphanate)
Disseminated Intravascular Coagulation (DIC)
Pathologic form of clotting that consumes large amounts of clotting factors
Causes widespread external and/or internal bleeding
Small clots form in microcirculation
DIC
Causes
Post partum hemorrhage
Abruption placentae
Amniotic fluid embolism
Dead fetus syndrome
Severe preeclampsia
Sepsis
Saline abortion
Acute fatty liver of pregnancy
DIC
Signs/Symptoms
Unusual bleeding
Spontaneous bleeding from gums or nose
Petechiae
Tachycardia
Diaphoresis
DIC
Treatment
Correct underlying cause

Volume replacement, blood component therapy, oxygen, continual reassessment

Resolution typically begins with birth of neonate
