Drug Classes for Psychoses
Phenothiazines: Most effective for hallucinations & delusions

Nonphenothiazines: less sedation & reduced anticholinergic effect, AE similar to Phenothiazines

Phenothiazines:
MOA: Blocks post synaptic dopamine receptors

AE: Sedation, Drowsiness, Sexual dysfunction, Hypotension, Parkinson-like symptoms, Spasms of back muscles, tongue & face, Dry mouth, Tachycardia, Tardive dyskinesia

Contraindications: Alcohol withdrawl, Coma, Brain damage, Bone marrow depression, Reye syndrome, Lactation

Nonphenothiazines:
Contraindications: Parkinson disease, seizure disorders, alcohol abuse, severe depression

Route: Oral, IM, IV

*benztropine mesylate/Cogentin to treat acute dystonia*

beta-adrenergic blockers & benzodiazepines for akathisia
Atypical Antipsychoses for Psychoses (Drug Therapy)
*DRUGS OF CHOICE

MOA: largely unknown, thought to block D2 Receptors, Serotonin, & alpha-adrenergic receptors

AE: Weight gain, Diabetes, Hypertriglyceridemia, increase risk of cerebrovascular events

Contraindications: Underlying cardiovascular disease, Seizure disorder, suicidal ideas, kidney/liver disease

-Treat both positive & negative symptoms associated w/ schizophrenia
-have fewer AE than Phenothiazines and Nonphenothiazines

MOA: Blocks dopamine & serotonin receptors

AE: HA, Insomnia, Dizziness, Tremor, Tardive dyskinesia, Blurred vision, Akathisia, Orthostatic Hypotension, Nausea/Vomiting, Diarrhea, Rash, Cough, Increase suicide risk, Esophageal motility

Contraindications: Lactation, Pregnancy, Cerebral vascular disease, Seizure disorders, Alzheimer dementia, Dysphagia, Parkinsonism
