Clinical Reasoning |
Thought process that therapist use to evaluate clients and design and carry out interventions. |
|
Three Elements of Clinical Reasoning |
1. Scientific - What are the possible things that can be done for this pt? - Demands careful and accurate assessment, analysis, and recording of info to make decisions
2. Ethical - What should be done for this client? - What is the right and fair path to take? - What is important for the client?
3. Artistic - What is the right action to take for this client? - Customizing right thing for pt. based upon our interpretation |
|
Clinical Reasoning Strategies |
1. Procedural - Focuses on the pt.'s disease or disability, and determines the most appropriate modality to use to improve clients fxnal performance - Tasks include problem identification, goal setting, and treatment planning
2. Interactive - Takes place during face-to-face interactions in getting to know the patient
3. Conditional - Looking at the pt.'s condition as a whole - What it means to the person, the physical context, social, and consideration of how the pt.'s condition may change depending on the level of participation in therapy
4. Narrative - Use of storytelling - Therapist tells stories to pts about other pts, and this may help the pt envision what the future holds for them
5. Pragmatic - How personal context may affect intervention, such as therapists repertoire of skills, resources available, and personal motivation |
|
Stages of Clinical Reasoning Development |
1. Novice - Uses knowledge from coursework
2. Advanced Beginner - Recognizes additional cues and begins to see pt. as an individual
3. Competent - Sees more facts, understands clients problems, individualizes tx - May lack creativity and flexibility at this point
4. Proficient - Views situations as a whole instead of parts - Able to develop a vision where pt. should go and modify easily
5. Expert - Recognizes and understands rules of practice - Uses intuition to know what to do next - Uses conditional reasoning |
