Why are PIs such a pain in the ass?
-Highly protein bound, poor bioavailability

What is the recommended regime in drug naive pts?
NNRTI based
Efavirenz + (AZT/Tenofovir) + (3TC/Emtricitabine)

PI based
Kaletra + AZT + (3TC/Emtricitabine)

Triple NRTI
ABC + 3TC + AZT
(only if first 2 cant be used)
What agents should NEVER be used in pregnant women?
Efavirenz

When should pregnant women/infants start/stop using tx?
C section benefits all women and kids
tx should start at 14 weeks. If earlier stop tx then begin again at 14 wks
Baby should begin wi 6-12 hrs of birth
What do we do when there is Hep B or C co-infection?
If you want to tx both HIV and HepB --->IFNa or b, 3TC

if only HepB tx is desired --->ADEFOVIR

With Hep C, may hold until CD4 is >200, then IFN + Ribavarin
What are some of the major consequences of long term antiviral therapy?
