Migraine
UNILATERAL, THROBBING, VISUAL PRODROMA, SENS TO NOISE AND LIGHT, NAUSEA, VOMITTING, FEMALES, AGE 7-14, GENETIC
Cluster
20 TO 30 YEARS OLD = ONSET, INTENSE PAIN (10/10), HORNERS IN 20%, ALCOHOL TRIGGER, 90% MALES , MAY CAUSE PATIENT TO WAKE UP IN NIGHT
Acephalgic
Similar to migraines except: can be bilateral or unilateral, no auras seen, kids have motion sickness, abdominal pain, fever, diarrhea, mood changes
Hypertensive
Hypertension, Systolic > 200, Diastolic > 110
Toxic
Drank too much coffee (etc), brought on by presence or withdrawal of substance
Ischemic Cerebrovascular
Usually >40 y/o, located ipsi. to occluded vessel in ICA, STROKE signs/symptoms, transient monocular blindness
Traction and Inflammatory
Location: Suboccipital (on neck, behind head), emesis upon waking up (throw up when awaken), aura lasts longer than pain phase, nuchal rigidity
Mass Lesion and Brain Tumors
Diagnosis of exclusion, waxes/wanes early in disease but later becomes constant, WORSE IN MORNING
Pseudotumor cerebri
OVERWEIGHT WHITE FEMALE ON BIRTHCONTROL PILL, usually under 30, worse in AM
Sinus
Location: maxillary front behind eyes, occurs duuring sinusitis, pressure or fullness, blood in nasal passage/communicating sinus (nasal drip), get fever whenever bad infection
Meningitis
Associated with fever, valsalva maneuver (such as when try to defecate/cough), napal-nuchal rigidity, intermittent syncope, infection
Temporal arteritis
Usually over 60, location: temporal but may spread to scalp, jaw pain, vision loss (see only top or bottom part), weight loss, jaw/scalp/shoulder, may hurt to brush hair
Raeder's Paratrigeminal Neuralgia
Headache pain not always present, sporadic (lasts only seconds to minutes), stabbing tic-like neuralgic pain, horners in 100%
muscle contraction
stress headache, location: hatband (feels like squeezing head), frontal, neck, shoulders, people with this are poor sleepers, triggered by anxiety
Conversion and post traumatic cephalgia
Following traumatic event, or emotional/stressful event
Asthenopia
Un or under corrected refractive error, nausea, accomodative dysfunction or binocular incoordination
