ATROPINE

Generic Name
Atropine sulfate
Atropine

Trade Name
Atropine
Atropine

Classification
Antiarrhythmic
Anticholinergic
Antidote
Atropine

Indications
<Hemodynamically significant bradycardia
<Asystole
<Organophosphate poisioning or nerve agent
<PEA with slow complex rate
Atropine

Contraindications
<Hypersensitivity to the drug
<Glaucoma
<Obstructive disease of the GI or GU tracts
Atropine

Adverse effects
Postural hypotension
Blurred vision
Dryness of the mouth,
GI reflux,
Nausea/vomitting,
Paraltic ileus,
Tachyarrhythmias
Urinary retention
Atropine

How supplied
.1mg/ml total of 10ml to equal 1mg of atropine, multidose vial 20 ml with .4mg/ml to total of 8mg
Atropine

Adult Dosage
Adult: Bradycardia, .5mg to 1mg. IV every three to five minutes as needed up to total of 3mg. In asystole give 1mg. IV, repeat every 3 to 5 minutes up to a total of 3mg.
Atropine

Peds Dosage
Bradycardia/aysystole give .02mg/kg IV/IO/ET up to .5mg for child or 1.0mg for adolescent. (MIN Dose .1mg) May be repeated once in 5 minutes.
Atropine

Organophosphate Poisioning Dosage
2mg repeat until BP and HR are stable.
Atropine

Precautions
Use caution in presence of myocardial ischemia and hypoxia. Avoid in hypothermic bradycardia. Usually not effective in second degree block type II and third degree blocks with wide QRS complexes. Antacids decrease absorption of med.
Epinephrine

Generic Name
Epinephrine Hydrochloride
Epinephrine

Trade Name
Adrenalin
Epinephrine

Classification
Sympathomimetic
Cardiac stimulant
Bronchodilator
Antiallergic
vaspressor.
Epinephrine

Action
<Stimulates alpha and beta-adrenergic receptors with the sympathetic nervous system.
< A potent cardiac stimulant, it stregthens the myocardial contraction and increase cardiac rate.
<Increases myocardial and cerebral blood flow during CPR
EPI strengthens the heart to contract harder and faster. Vasoconstriction effect that causes more blood flow during COR for the heart and brain.
Epinephrine

Indications
Cardiac Arrest
VF
Pulseless VT
Asystole
PEA
Anaphylaxis
Asthma (COPD)
Epinephrine

Contraindications
Angle-Closure glaucoma
Shock
Organic brain damage
Cerebral Arteriosclerosis
Labor and Delivery
Epinephrine

Adverse Effects
Nervousness, tremor, headace, agitation, dizziness, weakness, cerebral hemorrhage, palpitations, hypertension, tachycardia, anginal pain, nausea/vomiting and dyspnea
Epinephrine

How Supplied
Prefilled syringe .1mg/ml (1:10000) total of 10cc= 1mg

Multidose vial 1mg/ml (1:1000) total of 10cc
Epinephrine

Adult Dosage
Cardiac Arrest: 1mg (10 ml of 1:10000) every 3-5 mins during resuscitation. Tracheal route: 2 mg diluted in Saline
Anaphylaxis: 0.3-0.5mg (1-1000) IM or SQ
0.3-0.5mg (1:10000) IV if hypotension or poor perfusion
Asthma:Severe bronchoconstriction 0.3-0.5 mg SQ(1:1000)
Epinephrine

Peds Dosage
Cardiac arrest: (1:10000) Give 0.01mg/kg up to 1mg IV,IO

Anaphylaxis:(1:1000)0.01mg/kg SQ or IM up to 0.3mg if patient was exposed to commonly recognized allergen and has respiratory distress or hypotension
Epinephrine

Precautions
High does do not improve survival or neurologic outcome and may contribute to postresuscitation myocardial dysfunction. Raising blood pressure and increasing heart rate may cause myocardial ischemi, angina and increased myocardial oxygen demand. Higher does maybe required to treat poison/drug induced shock. Do not use concurrently with Brevibloc. The effects of the maybe potentiated by tricyclic antidepressants.
Amiodarone

Generic Name
Amiodarone Hydrochloride
Amiodarone

Trade Name
Cardarone
Amiodarone

Classification
Antiarrhythmic
Amiodarone

Actions
>Block sodium channels at rapid pacing frequencies
>decreases sinus rate
>increases PR and QT intervals
>After IV use, Amiodarone relaxes vascular smooth muscle
>Reduces peripheral vascular resistance (afterload)
Amiodarone

Indications
>VF and Pulseless VT refractory to Defib
>Stable VT and unstable VT refractory to cardioversion
>Conversion of atrial arrhythmias after rate control
Amiodarone

Contraindications
>Hypersensitivity
>Cardiogenic shock
>bradycardia
>AV blocks
>Liver disease
Amiodarone

Adverse effects
Cough and progressive dyspnea, worsening of arrhythmias, symptomatic bradycardia, sinus arrest, SA node dysfunction, CHF edema, hypotension, cardiac conduction abnormalities, cardiac arrest, abnormal involuntary movements, headache, N/V, abdominal pain, flushing and shock
Amiodarone

How supplied
Injection: 50 mg/ml vial
Amiodarone

Adult Dosage
>Pulseless VF/VT: 300 mg IV
Consider repeat 150 in 3-5 mins
Post resuscitation maintenance dose: 1mg/min IV/IO 6 hrs
>Wide complex tachycardia(stable): 150mg rapid infusion (15 mg/min) over first 10 min.
May repeat 150 mg rapid infusion IV every 10 minutes as needed.
MAX DOSE: 2g IV/IO over 24 hours
Amiodarone

Ped Dose
Pulseless VF/VT: 5mg/kg IV/IO bolus. Perfusing VT: 5mg/kg IV/IO over 20-60 min

MAX DOSE: 15mg/kg per IV per day
Amiodarone

Precautions
May produce vasodilation and hypotension. May have negative inotropic effects. My prolong QT interval. Use with caution if renal failure is present
