Indications
|
A. treatment of poisoning due to organophosphates or nerve gas exposure
B. depends on the severity of symptoms 1. Mild Symptoms -H/A -N/V -ABD cramps -Diarrhea 2. Moderate symptoms -generalized muscle weakness and twitching -slurred speech -pinpoint pupils -excessive secretions -SOB 3. Severe symptoms -seizures -skeletal muscle paralysis -respiratory failure -coma |

Precautions and side effects
|
A. short acting drug, repeat dosing may be neccessary
B. Dizziness, blurred vision, diplopia, H/A, drowsiness, nasuea, tachycardia and muscle weakness have been reported C. Rapid IV = tachycardia, laryngospasm, muscle rigidity, transient neuromuscular blockade -SLOWLY INFUSE |

A. Pralidoxime 600mg/2ml in one autoinjector contained in mark 1 kit
B. pralidoxime 1g injectible 20 ml vials C. sterile water for injection, use to reconstitute pralidoxime powdered form 20 ml vials |

Administration
|
Adults by IM/IV
STANDING ORDER Dosage depending on severity of symptoms, administer if weakness or paralysis present 1. Moderate Symptoms 2 autoinjectors or (1200 mg) 2. Severe Symptoms 3 autoinjectors (1800 mg) PEDS by IV MD ORDER 25-50 mg/kg @ 4mg/min -no more then 0.1 cc/min in kids |

Routes of administration
|
IM using the mark I autoinjector (600mg/2ml)
a. when administering IM, autoinjector shuld be held in place for 10 seconds b. inject into lateral thigh or upper outer buttock using multiple injection sites IV infusion: reconsititute 1.0g powder vial with 20 mL sterile water for injection a. ADULTS = no more then 200mg/min (4cc/min) CHILDREN = no more then 4mg/min (0.1 cc/min) b. rapid IV injection may cause tachycardia, laryngospasm, muscle rigidity, transient NM blockade... SLOW MOFO |

no contraindication to the use of paralytics in patients who have OG poisoning, however suxx and vec will have a longer effect
