What tool should you use when suspecting or detecting child abuse and/or neglect?

Suspected, detected or disclosed cases of child abuse must be acted on and reported using the '6 step protection intervention'

What are the 6 steps?

1) identification of signs and symptoms


2) validation and support


3) health and risk assessment


4) intervention/safety planning


5) referral and follow up

What steps are taken under 'step 1: identification of signs and symptoms'

- observe caregiver-child interaction


- take a history from the parents and caregivers


- if child has an injury ask them about the injury using open-ended, non-leading questions


- past history: review past records for history of abuse/suspected abuse/alerts


- social history


- physical examination


- complete child protection checklist for children under 2 years

What steps are taken under step 2 'support and validation'?

For the child:


- Listen to what you are being told


- do not put words in a child's mouth


- allow them to tell only as much as they want


- act on the assumption that they are telling the truth


For the adult


- listen to what you are being told


- thank them for telling you if appropriate


- let them know that you will act to keep them safe

What steps are taken under Step 3 'Health and Risk Assesment'?

- mental health assessment


- family and environmental contact e.g. Intimate partner violence, alcohol and substance abuse, untreated mental health


- risk to child or young person or risk to other children/young people


- treat injuries (if applicable)

What steps are taken under step 4 'safety planning'?

- children admitted to hospital with actual or suspected abuse or neglect should be managed in accordance to the memorandum of understanding between DHBs, CYF, and the police


- if child is being harmed and/or safety concerns warrant a statutory intervention consult with experienced colleague and/or MDT prior to referral


- if child is at risk but concerns don't warrant a statutory intervention consult with experienced colleague and/or MDT prior to referral


- if untreated mental health or alcohol/drug abuse consult and refer


- if intimate partner violence is disclose consult and refer

What steps are taken under step 4 'safety planning'?

child being harmed


- report concern to CYFS and/or police


- manage in accordance to the memorandum of understanding between DHBs, CYF, and Police


if child is at risk (but not needing statutory response)


- discuss with experienced colleague, CYF, or MDT

What steps are taken under step 5 'referral(s)'?

- if child is admitted to hospital with actual or suspected abuse refer to police and/or CYFS


- if child is at risk a statutory response is not needed refer to family support service


- if untreated mental health or alcohol/drug abuse refer to appropriate mental health agency


- if IPV disclosed refer to specialist services (e.g. Women's refuge, CYFS)

What steps are taken under step 6 'document'?

- Document intervention (history taken, examination of findings, consultation process, risk assessment, and referrals)


- a written report of concern is required when making a referral to CYFS
