Mission statement

Vision statement

Action or behavioral objective
Describes the behaviors or actions in which the priority population will engage in that will resolve the health problems and move you toward achieving the program goals

Outcome or program objective
The ultimate objectives of a program and are aimed at changes in health status, social benefits, risk factors, or quality of life,
SMART objective
Objectives that are Specific, Measureable, achievable, realistic, and time phased
A well written objective
Outcome (what) + Priority population (who) + Conditions (when) + criterion (how much)
attitude objective
those that describe the desired attitude of those in the population
awareness objective
Those that describe of what those in the priority population will become aware
conditions
a major component of an objective that describes when or how the outcome will be observed
criterion
major component of the objective that describes how much change will occur
impact objective
A category of objectives comprised of the four types of learning objectives (awareness, knowledge, attitudes, and skills), action, behavioral, and environmental objectives.
