Obtaining smear from Vaginal Pool
Rub blunt end of an Ayre spatula over veginal wall under and lateral to cervix

If mucosa is dry, moisten sterile swab with normal saline solution
Cervical Scrape
Insert bifid end of Ayre spatula into vegina with more pointed bump into cervical os. Rotate all the way.

This scrapes surface of squamocolumnar junction and cervix.

Specimen important for adolescent chose endocervical cells haven't yet migrated into endocervical canal
Endocervical Specimen
Insert cytobrush into the os. Rotate 720 degrees in one direction

Cytobrush gives higher yield of endocervical cells at squamocolumnar junction

Herpes Simplex Virus (Type 2)
Episodes of local pain, dysuria, fever
Clusters of small, shallow vesicles with surrounding erythema. Erupt on genital areas and thighs. Inguinal adenopathy, edema, vesicles on labia rupture in 1-3 days leaving painful ulcers. Remains dormant indefinitely.

Initial infxn lasts 7- to 10 days and recurrent infections last 3-10 days
HPV (genital warts)
Painless warty growths may go unnoticed by woman

Pink, flesh colored, soft, pointed, moist, warty papules, single or multiple in cauliflower patch. Occur around vulva, introitus, anus, vagina, cervix
Uterine prolapse
With straining or standing, uterus protrudes into vagina

1st degree: cervix appears at introitus w/straining
2nd degree: cervix bulges outside introitus with straining
3rd degree: whole uterus protrudes even without straining
Bluish cervix
Cyanosis, normal with pregnancy and other conditions causing hypoxia or venous congestion
Carcinoma
Bleeding between periods or after menopause, unusual discharge, ulcer and induration noted and diagnosed by pap smear and biopsy
Candidiasis (Moniliasis)
Intense itching, thick whitish discharge
Vulva & vagina are erythematous and edmatous, discharge thick white curdy like cheese, diagnosed by microscopic examination

Predisposing causes: oral contraceptives or antibiotics, alkaline vaginal pH, pregnancy
