Testis
Produces sperm. One in each scrotal half of scrotum separated by a septum. Solid oval shaped, suspended by spermatic cord.

Left testis is lower than the right because the left spermatic cord is longer
Epididymis
Caps the testis and is a coiled system and main storage site of sperm.
Vas Deferens
Approximates with other vessels to form spermatic cord and later separates and joins the duct of seminal vesicle to form ejaculatory duct
Ejaculatory Duct

Syphilitic Chancre
Small, solitary papule erodes to red, round, or oval superficial ulcer with yellowish serous discharge

Begins with 2-4 weeks of infection

Palpation: nontender indurated base that can be lifted like a buttom between thumb and finger. Lymph nodes enlarge early but are nontender
Condylomata Acuminata (genital warts)
soft, pointed, moist, fleshy, painless papules

may be single or multiple in cauliflower-like patch.

gray, pale yellow, or pink ( in white males)

black or translucent gray (in black males)

on shaft of penis, behind corona, or around anus

caused by HPV and is a common STD
Carcinoma
Red, raised, warty growth or ulcer with watery discharge. May grow necrose and slough. Painless. Almost always on glans or inner lip of foreskin and follows chronic inflammation

Enlarged lymph nodes
Urethritis
purulent urethral discharge and dysuria, reddened meatus edges, everted, swollen. Cloudy urine
Priapism
Painful erection of penis without sexual desire. Occurs with sickle-cell trait or disease, leukemia, malignancy, or local trauma, or spinal cord injury
Urethral Stricture
Pinpoint, constricted opening at meatus or inside along urethra. Congenital or due to urethral injury. Gradual decrease in force and caliber of urine stream is most common symptom. Shaft feels indurated along ventral aspect.
Testicular Torsion
Sudden excruciating pain in testicle. May also have lower abdominal pain, nausea, and vomiting.

Inspection: red, swollen scrotum, one testis (usually left) higher owing to rotation and shortening

Palpation: cord feels thick, swollen, tender, cremasteric reflex absent on side of torsion

It is sudden twisting of spermatic cord usually on left side due to faulty anchoring of testis on wall of scrotum allowing testis to rotate. Blood supply is cut off.
Spermatic cord Varicocele
Looks like a bag of worms

Dull pain, constant pulling or dragging feeling
Soft irregular mass posterior to and above testis, collapses when supine. Distinctive like a bag of warms

Dilated, tortuous varicose vein in spermatic cord due to incompetent valves in vein. Most often on left side and common in young males
Hydrocele:
Painless swelling, enlarged mass that transilluminates with pink or red glow. Nontender mass, can get fingers above mass

Cystic, circumscribed collection of serous fluid in tunica vaginalis surrounding testis
Scrotal Hernia
Due to indirect inguinal hernia;

swelling, pain, enlarged, doesn't transilluminate, soft mushy mass, cant get fingers above mass
