[Upper:]
Acute pyelonephritis
Acute bacterial prostatitis
[-Lower:]
Acute cystitis
Acute urethral syndrome
-Recurrent urinary tract infections (UTIs)

Trimethoprim/sulfamethoxazole is frequently the treatment of choice for oral therapy of UTIs.
Fluoroquinolones often used as well
Urinary Tract Antiseptics:
Nitrofurantoin [Furadantin]
Methenamine [Mandelamine]
Nalidixic acid [NegGram]
Cinoxacin [Cinobac]
Methenamine [Mandelamine]
[Overview]
[Uses]
[AV]
*Has multiple actions*
Decomposes into ammonia and formaldehyde, which denature bacteria

Uses: chronic lower UTIs

Adverse effects:
SAFE
Well tolerated
Contraindicated in?
Renal patient
– Liver dysfunction
Nitrofurantoin

[Action]
*Has multiple actions*
-Nitrofurantoin is activated by bacterial enzymes, = inactivates proteins.
-Inactivations cause the inhibition of biochemical processes of aerobic energy metabolism and the syntheses of bacterial DNA,RNA, cell wall, and protein.

-Nitrofurantoin interferes with a variety of bacterial processes, this may explain the lack of acquired bacterial resistance.

-Typically used only in UTI, and UTI prophylaxis
