What is the most powerful tool in psychotherapy and the only one that actually works for the reasons it claims to work & for the disorder's it was designed to treat?
Systematic Desensitization
What is Systematic Desensitization used to treat?
types of anxiety disorders
some depressive disorders
stress-related disorders
_________ developed systematic desensitization while _____ extended it to a coping skill

Bourne
Meichenbaum
Wolpe
Wolpe developed it & Meichenbaum extended it
What are the 3 stages in systematic desensitization?
1. relaxation
2. in vitro exposure
3. in vivo exposure

Every time anxiety develops, relaxation exercises ensue
Why is depression underdiagnosed?
1. various cultural taboos
2. statistics don't include milder forms or when comorbid w/another disorder
What is the triad of cognitive distortions that underlie most depressions?
1. polar reasoning
2. selective abstraction (look for confirmation of the negative)
3. Overgeneralization
The depressive affect & vegetative signs appear before the dysfunctional thoughts. This means......
you should suspect a physical disorder
Naming cognitive distortions:

"I'll never be good enough"
Black/white thinking/polarization/permanence
Naming cognitive distortions

"Everything is the same - useless"
over-generalization
Naming cognitive distortions

"I can't do anything right"

defectiveness/shame of inadequacy
Naming cognitive distortions

"no one really knows how hard it is for me"
aloneness/social isolation/ alienation
What is Progressive Relaxation Training?
Jacobson's system of relaxation training; involves tensing & relaxing 32 muscle groups w/verbal instructions for the client to attend to the sensations of both tension & relaxation
What is the rationale behind Progressive Relaxation Training?
1. Ppl become more aware of when they aren't relaxed
2. They now have a method of intervening before they get too tense
What is the signal breath?
a relaxation technique used for everything from pain management to anger control
How do you address anxiety using CBT?
1. Find out distortions/schema around feared situation
2. Find out how they physically feel anxiety
3. Determine client's current coping methods, perceived efficacy, intrusiveness, & effects it's having on their lifestyle
4. Determine how client usually relaxes and/or explore new alternatives

Desensitization!!
What is the difference between relaxation protocols and hypnosis protocols?
Hypnotic suggestion uses a command "you are feeling" whereas relaxation instruction simply calls attention to your physical sensation.
What is paradoxical relaxation?
During PMR some clients experience a sudden inc in tension due to feelings of panic/loss of control
Who should you not use PMR with?
-clients w/a physical disorder better treated by meds/surgery
-clients w/a condition that reqs strengthening rather than relaxing (low back pain from atrophy/disuse)
-client is on sedatives or muscle relaxants
-client w/low blood pressure, kidney disease
How should you speak/pace yourself while doing PMR?
-speak differently during "tense" phases and "relaxation" phases
-tense:relaxation should be about 5sec: 35sec
- do each muscle gp 2x
-do NOT use technical terms, "stress" "effort" or "tension" during relaxation phase
-be direct NOT wishy washy
-debrief after
