Pharmagolic: water soluble vitamins
Indications: Treatment & prevention of Vit C deficiency (scurvy) with dietary supplementation. Supplementary in some GI diseases during long-tem parenteral nutrition & chronic hemodialysis.
Route/Dosage: PO (adults): scurvy-----500mg/day for @least 14 days. Prevention of deficiency-----50-100mg/day. IM (adults): scurvy-----100-500mg/day for atleast 14 days. IV (adults & children): Prevention of defiency-----determined by need.

Therpeutic: antianginals, antihypertensives
Pharmacologic: beta blockers
Indications: management of hypertension. Management of angina pectoris, Prevention of MI.
Route/Dosage: PO (adults) Antianginal----50mg once daily; maybe increase after 1 wk to 100mg/day (up to 200mg/day). Antihypertensive: 25-50mg once daily; maybe increase after 2wk to 50-100mg once daily.
Adverse Reactions/Side Effects: CNS: fatigue, weakness, anxiety, depression, dizziness, insomnia.
Nursing Implications: monitor BP, ECG & pulse frequently during dosage adjustment peroid & periodically throughout therapy. Monitor I&O ratios & daily weights. assess routinely for CHF 9dyspnea, rales/crackles, wt gain, peripheral edema & JVD).

Trade name: Apresoline, Novo-Hylazin
Therapeutic: antihypertensives
Pharmacologic: vasodilators
Indications: Moderate to severe hypertension (with a diuretics).
Route/Dosage: PO (adults): hypertension----10mg 4 times daily initially. After 2-4 days may increase to 25mg 4 times daily for the rest of the 1st wk; may then increase to 50mg 4 times daily (up to 300mg/day). Once maintenance dose is established, 2 daily dosing may be used. CHF:----25-37.5mg 4 times daily; maybe increased uo to 300mg/day in 3-4 divided doses.
Adverse Reactions/Side Effects: CNS: dizziness, drowsiness, lethargy, hangover, headache, ataxia, slurred speech, forgetfulness, confusion.
Nursing Implications: conduct regular assessment of continued need for treatment. GERI: assess geriatric patients carefully for CNS reactions as they are sensitive. assess fall risk.

Indications: Mild to moderate dementia associated with Alzheimer"s disease.

Adverse Reactions/Side Effects: CNS: abnormal thinking, amnesia, behavior changes, daytime drowsiness.
Nursing Implications: assess mental status, sleep patterns & potentail abuse before administration. assess alertness, @ time of peak effect.

Indications: PO, Transdermal: Management of mild to moderate hypertension. Epidural: management of cancer pain unresponsive to oipoids alone
Route/Dosage: PO (adults): hypertension (initial dose)----100mcg (0.1mg) bid, increase by 100-200mcg (0.1-0.2mg)/day q 2-4 days. PO (geriatric pts) : 100mcg (0.1mg) 2 bedtime initially, increased as needed.
Adverse Reactions/Side Effects: CNS: drowsiness, depression, dizziness, nervousness, nightmares, dry mouth, constipation, N/V.
Nursing Implications: Monitor I&O ratios & daily weight. assess for edema daily especially at beginning of therapy. Monitor BP & pulse frequently. Pain: assess location, character & intensity of pain prior to, frequently during the first few day & routinely.

Indications: Prophylaxis & treatment of: Venous trombosis, pulmonary edema, atrial fibrillation,with embolization. Management of myocardial infarction.Decrease death, subsequent MI, future tromboembolic events.
Route/Dosage: PO,Iv (adults): 2.5 -10mg/day for 2-4 days, then adjust daily dose by results of prothrombin time or INR. Initiate therapy with lower dose in geriatric or debilitated pts.
Adverse Reaction/Side Effects: GI: cramps, nausea, dermal necrosis, bleeding, fever.
Nursing Implications: assess pts for signs of bleeding & hemorrhage (bleeding gums, nosebleeds, unusual bruising, tarry black stools, hematuria, fall in hematocrit or BP, guaiac-positive stools, urine, or nasogastric aspirate).

Indications: Alone or with other agents in the management of hypertension. Treatment of diabetic nephropathy in pts with type 2 diabetes. Prevention of stroke in pts with hypertension and left ventricular hypertrophy.
Route/Dosage: PO (adults): Hypertension----50mg once daily initially (range 25-100mg/day as single daily dose or 2 divided doses) (initiate therapy at 25 mg once daily in pts who are receiving diuretics or are volume depleted).
Adverse Reactions/Side Effects: dizziness, fatigue, headache, insomnia, weakness, chestpain, edema, nasal congestion, diarrhea.
Nursing Implications: assess BP (lying, sitting, standing) & pulse periodically during therapy. assess for signs of angioedema (dyspnea, facial swelling).

Nursing Implications: assess for abdominal distention, presence of bowel sounds & usual pattern of bowel function. assess color, consistency & amount of stool produced.

Therapeutic: antiarrhythmics (group IB), anticonvulsants
Pharmacologic: hydantoins
Indications: Treatment/prevention of tonic-clonic (grandmal) seizures & complex partial seizures.
Route/Dosage: Anticonvulsants: PO (adults): loading dose of 15-20mg/kg as extended capsules in 3 divided doses given every 2-4 hr; maintenance dose 5-6mg/kg/day given in 1-3 divided doses; usual dosing range=200-1200mg/day. Antiarrhythmics: IV (adults):50-100mg q 10-15 min until arrthytmia is abolished or a total of 15 mg/kg has been given or toxicity occurs. PO (adults): loading dose:250mg qid for 1 day, then 250mg bid for 2 days, then maintenance dose----5-10mg/kg/day in 2-3 divided doses IV or PO.
Adverse Reactions/Side Effects: ataxia, agitation, confusion, dizziness, diplopia, nystagmus, hypotension, gingival hyperplasia, nausea, rash.
Nursing Implications: assess oral hygiene. assess for phenytoin hypersensitivity syndrome (fever, skin rash, lymphadenopathy). Seizures: assess location, duration, frequency & characteristics of seizures activity.

Indications: PO: Prevention/treatment of iron-deficiency anemia.
Route/Dosage: PO(adults): Deficiency----120-240mg/day (2-3 mg/kg/day) in 2-4 divided doses. Prophylaxis----60-100mg/day).
Adverse Reactions/Side Effects: CNS: IM,IV- seizures, dizziness, headache, syncope. IM/IV: hypotension. PO: constipation, dark stools, diarrhea, epigastric pain, bleeding.
Nursing Implications: assess nutritional status & dietary history to determine possible cause of anemia & need for pt teaching. assess bowel function for constipation or diarrhea. assess pts s/s of anaphylaxis (rash, pruritus, laryngeal edema, wheezing).
