Blood flow through heart is regulated by

HR, metabolic activity of the heart, blood vessel tone (constriction)

Systemic =

Pulmonary =

S = L side, high pressure system

P = R side, low pressure system

Cardiac Glycosides: Action

Increased contractility of cardiac tissue; decreased preload, increased blood flow, decreased edema, promoting fluid excretion

Inhibits sodium-potassium pump, resulting in increased intracellular sodium, leading to increased calcium in cells

3 effects on heart muscle:

1. positive inotropic action (increases myocardial contraction stroke volume)

2. negative chronotropic action (decreases HR)

3. negative dromotropic action (decreases conduction of heart cells)

Cardiac Glycosides: Conditions

CHF management

A-fib & flutter, tachy

Cardiac Glycosides: Drugs

Digoxin (Lanoxin)

Cardiac Glycosides: Nursing

Low margin of safety

Serum digoxin = 0.5-2

Assess heart rate before

If on K+ wasting diuretic, increased risk for toxicity

Cardiac Glycosides: Side Effects

Bradycardia

Toxicity (flu symptoms)
