Blood flow through heart is regulated by |
HR, metabolic activity of the heart, blood vessel tone (constriction) |
|
Systemic = Pulmonary = |
S = L side, high pressure system P = R side, low pressure system |
|
Cardiac Glycosides: Action |
Increased contractility of cardiac tissue; decreased preload, increased blood flow, decreased edema, promoting fluid excretion
Inhibits sodium-potassium pump, resulting in increased intracellular sodium, leading to increased calcium in cells
3 effects on heart muscle: 1. positive inotropic action (increases myocardial contraction stroke volume) 2. negative chronotropic action (decreases HR) 3. negative dromotropic action (decreases conduction of heart cells) |
|
Cardiac Glycosides: Conditions |
CHF management A-fib & flutter, tachy |
|
Cardiac Glycosides: Drugs |
Digoxin (Lanoxin) |
|
Cardiac Glycosides: Nursing |
Low margin of safety Serum digoxin = 0.5-2 Assess heart rate before If on K+ wasting diuretic, increased risk for toxicity |
|
Cardiac Glycosides: Side Effects |
Bradycardia Toxicity (flu symptoms) |
