What is cardiogenic shock? |
Failure of the heart pump resulting in decreased cardiac output and thus decreased tissue/organ perfusion |
What is the most common cause of cardiogenic shock? |
Myocardial infarction |
What are the symptoms of cardiogenic shock? |
- pulmonary oedema (dyspnoea, crackle sound in lungs) - peripheral oedema (distended jugular vein oedema) - hypotension - chest pain - cool, clammy skin - oliguria |
What is the main underlying issue in cardiogenic shock? |
Decreased contractility of the heart muscle |
What medications could you give in cardiogenic shock? |
- diuretics (if pulmonary/peripheral oedema) - vasopressors which cause vasoconstriction - dopamine - dobutamine |
What type of drug is dopamine and dobutamine? |
Positive inotropics |
What do positive inotropic medications do? |
Increase heart contractility |
What is a side effect of dobutamine? |
Worsening hypotension because it causes vasodilation |
