What are the three types of Pericardial Dz?
Pericarditis, Pericardial constriction, and Tamponade.
4 types of "Dirt" for pericarditis?

Are etiologies always infectious?

Sx?

ECG findings?

Tx? (3)
Serofibrinous
- most common, plasma proteins
Serous
- mainly PMN, lymphocytes, Thin fluid
Purulent
- puss
Hemorrhagic

No, can be either infectous or non-infectous.

- Pleuritic chest pain (may radiate to back)
positional: worse lying down
- fever

Diffuse ST elevation: in random/varying leads...
PR depression due to abnormal atrial depolarization

NSAIDs, steroids, fluid drainage
Which kind of pericardial dz limits the expansion of the ventricles, and inhibits transmission of thoracic pressure into pericardial space.

Effects?

Sx? (2)

Classic sign on PE?
Constrictive.

Decreased PL because now the incoming blood rush with inspiration causes the RV to squish the LV.

\CO --> fatigue, dizziness
Venous congestion: Ascities, lower extremity edema

JVP elevation with inspiration = Kussmaul's sign.
Upon cardiac catheterization, which do you see on the Atrial wave form in constrictive pericarditis?

a-->x-->v-->y (this is the letter progression for the atrial wave form)

How about on a RV/LV tracing?
Inspiration/Expiration?

Tx?
"stadium analogy" w/o enough seats.
- blood rushes into fill ventricle, resulting in a steeper Y-wave descent.

"Sqrt sign" (dip and plateau) in ventricle tracing.
- there is a diastolic equalization of pressures
- there is a rapid filling until the ventricle hits the rigid pericardium and stops.

Inspiration increase RV pressure and decreases LV pressure b/c of previously discussed impingement.

Remove pericardium.
______ is basically an extreme form of cardiac constriction, resulting in extremely impaired ventricular filling.

See on PE? (give me the # mmHg drop necess for dx.)
Cardiac Tamponade.

Pulsus paradoxis
- decrease in systemic pressure with inspiration
***>10mmHg drop in sysP w/ inspiration = pulsus paradoxus
In Cardiac tamponade, what is seen on the atrial pressure recordings?

Tx?
- Blunted Y-descent b/c the blood cannot get into the left ventricle.
- diastolic equalization of pressure

Pericardialcentesis
