Characteristics of Borderline Personality Disorder

A persuasive pattern of instability and dysregulation across emotional, behavioral, cognitive and interpersonal domains.



Problems with anger and anger expression. Self destructive behavior

DBT

Uses the therapeutic relationship to point out ambivalence. Blends behavioral and crisis intervention theories. Emphasizes acceptance and tolerance balanced with change.

Contingency Procedures

Requires therapist to organize their own behaviors so they can influence the client. Must set limits, use punishment with great care in low doses.

Exposure and Dialectical Behaviors

Client is exposed to clues of previous traumatic events, and client learns hows to control the emotions.

Cognitive Modification

Message of DBT is that the source of the client's distress is the extremely stressful events of his life.

Telephone Consultation

Between session phone calls provide coaching in skills and promote skill generalization, provide emergency crisis intervention.

Enter the paradox

Clients have the right to kill themselves but if they ever convince the therapist that suicide is imminent, there will be a serious intervention.



Clients are not responsible for how they are but who they become.


Devil's advocate

Therapist argues in favor of the dysfunctional belief.



Client: I'm overweight, I'm better off dead.


SW: Perhaps I should kill myself because I'm 10 pounds overweight.

Extending

Extend the movements endpoint slightly further than what would naturally occur, leaving the challenger vulnerable and off balance.



ex: Patient needs to see therapist immediately or might end up killing themselves.



Your life is in danger. I will call the rescue squad immediately!

Utilize client's resistance. Therapeutic problems are an opportunity for therapist to help the client.
