Med: alendronate (generic) Fosamax (trade)
Dose:(postmenopausal) PO 10 mg (pagets) PO 40mg (preventive) PO 5mg (corticosteriod induced) 5mg
Action: decreases rate of bone resorption and may directly block dissolution of hydroxyapatite crystals of bone; inhibits normal & abnormal bone resorption, mineralization
Side effects: (severe) perforation, angioedema, Stevens-Johnson syndrome, toxic epidermal necrolysis (common) none listed
Contra: hypersensitvity to bisphosphnoates, delayed esophageal emptying, inability to sit or stand 30 min, hypocalcemia
Patient teaching/NSC: to remain upright for 30 min after dose to prevent inrritation, if dose is missed skip dose do not double dose, to take in am before food, other meds, take with 6-8oz of H2O only, to take calcium, vit D if instructed to by HCP, to use Wt. bearing exercise to > bone density, to inform HCP provider if pregnant or if planning to become pregnant or if breastfeeding
Med: prednisone (generic) Apo-prednisone, Deltasone, Liquid Pred, Meticorten, Orasone, Panasol-S, Prednicen-M, Sterapred, Winpred (trade)
Dose: PO 5-60mg, (MS) Po 200 mg
Action: < inflammation, by supression of migration of polymorphonuclear leukocytes, fibroblasts, reversal to capillary permeability, & lysosomal stabilization, minimal mineralocorticoid activity
side effects: (severe) circulatory collapse, thrombophlebitis, embolism, GI hemorrage, thrombocytopenia (common) none noted
Contra: Psychosis, hypersensitivity, idiopathic thrombocytopenia, acute glomerulonephritis, amebiasis, fungal infections, nonasthmatic bronchial disease, child<2yr, AIDS, TB
Patient teaching/NSC: that emergency ID as steroid user should be carried, to notify HCP if theraputic response <, to avoid vaccinations, not to disscontinue abruptly or adrenal crisis can result, to avoid OTC products- cold meds, that the product cause immunosuppression & to report any signs of infection, the symptoms of adrenal insufficiency.

Med: ibuprofen (generic) Advil, Bayer, Motrin, Excedrin, Menadol, Midol, Novoprofen (trade)
Dose: (analgesic) PO 200-400mg ( dysmenorrhea) PO 400mg, (antiinflammatory) PO 300-800mg
Action: inhibits prostaglandin synthesis by < enzyme needed for biosynthesis: analgesic, antiinflammatory, antipyretic
Side effects: (severe) CV thrombotic events, MI, stroke, hepatitis, GI bleeding, ulceration, nephrotoxicity, blood dyscrasias, nectrotizing fasciitis, anaphylaxis, Steven-Johnsons, (common) headache, anorexia
Contra: hypersensitivity, asthma, severe renal/hepatic disease, avoid in 2nd-3rd trimester of pregnancy, perioperative pain in CABG
Patient teaching/NSC: to report blurred vision, ringing, roaring in ears; may indicate toxicity; eye/ hearing test are needed with long term use, avoid driving if drowsiness or dizziness occurs, ! report changes in urinary pattern, > wt., edema, > pain in joints, fever, blood in urine; indicate nephrotoxicity, theraputic inflammatory effects take up to 1mo, 1 to avoid alcohol, NSAIDs, salicylates; bleeding may occur, to use sunscreen to prevent photosensitivity.
Med: Aspirin (generic) Acuprin, A.S.A, Bayer, Empirn, Halfprin, Novasen (trade)
Dose: (arthritis) PO 2.6, (pain/fever & thromboembolic disorders) PO/RECT 325-650mg, (rheumatic fever) PO 5-6g/day, (transient ischemic attacks & MI/stroke pervention)PO 160-325mg/day, (CABG & PTCA) Po 325mg,
Action: blocks pain impulses in CNS, reduces inflammation by inhibition of postaglandin synthesis; antipyretic action results from vasodilation of peripheral vessels; < platelet aggregation.
Side effsects: (severe) GI bleeding, seizures, coma, hepatitis, thrombocytopenia, aranulocytosis, leukopenia, neutropenia, hemolytic,
Side effect cont.:Reye's sydrome (chidren), anaphylaxis, laryngeal edema
(common) nausea, vomitting, rash.
Contr: pregnancy 3rd trimester, hypersensitivity, tartrazine, Gi bleeding, NSAIDs, bleeding disorders, child <12yr, child with flu like symptoms, breastfeeding, vit K deficiency, peptic ulcer.
Patient teaching/NSC: to report any symptoms of hepatotoxicity, renal toxicity, visual changes, ototoxicity, allergic reactions, bleeding. to avoid if allergic to tartrazine, not to exceed recommended dose, to read OTC meds they may also contain Aspirin, for arthritis effect may take 2wk, to report tinnitus, confusion, diarrhea, sweating, hyperventilation, to avoid alcohol use, to discard tabs if vineger like smell is detected,
