Macrocytic anemia with anisocytosis

Interpretation:
Increased indirect (unconjugated) bilirubin and LDH indicates hemolytic anemia (RBC lysis/increased destruction of RBC's)
Reticulocyte count (increased)
Coombs' test: Positive (IgG)
Peripheral smear: Marked polychromasia, many spherocytes
Interpretation:
Increased retic count means the bone marrow response to blood loss is normal. Positive Coombs' test means that patient's RBCs have antibodies and/or complement bound to them; RBC's are cross-linked by multivalent Abs and this causes clumping. Marked polychromaisa shows increased retics present (nuclear material is polychromatic) and many spherocytes indicates that RBC's are spherical, less pliable b/c of splenic phagocytosis; part of RBC membrance is lost. Normal shape is biconcave disc.
