Acute tubular necrosis
-characterized by death of cells that line the renal tubules; usually is focal of localized to distinct areas
-results in renal tubule dysfunction
-is the most common result of pre-renal acute failure, whereby too little blood is delivered to the kidney and too little fluid reaches the renal tubules
ATN
most common result of intrinsic acute renal failure
ATN
pathognomonic for ATN
muddy brown casts on urinalysis
some cases are associated with (2)
hypoperfusion and destruction of renal tubules secondary to toxins
Toxic Causes of ATN (7)
free hemoglobin (from massive hemolysis)
myoglobin (from rhabdomyolysis)
Ethylene glycol (radiator fluid)
Aminoglycosides
Amphotericin B
Cisplatin
Contrast Media
pts at risk for ATN
should be pre-treated with IV fluids before receiving IV contrast
pts on metformin should have metformin withheld for 48hrsin advanced of contrast admin since if ATN occurs the metformin will not be excreted and serum levels of metformin will rise, resulting in potentially life-threatening lactic acidosis
Adressing ATN promptly; not addressed promptly
most pts fully recover within 7-21 days;
it can cause chronic renal failure
