Atypical Pneumonia

- atypical presentation


- organisms not detectable on Gram stain


- organisms cannot be cultured.



Epidemiology

- children


- adolescents


- young adults


- specific RFs

Symptoms

- 3-4 day prodrome of malaise, headache, fever, dry cough


- constitutional Sx usually predominate over respiratory findings.


- may have moderate amounts of sputum



Signs

- typically sparse



Lab Findings

- typically limited


- need high level of suspicion


- little or no consolidation


- small increases in WCC


- minimal if any alveolar exudate

Pathogens

1. Mcyoplasma pneumoniae (70%)

2. Chlamydia pneumoniae


3. Legionella pneumophilia


4. Influenza A and B esp during epidemics


5. Adenovirus



Radiology

- typically diffuse


- interstitial pattern

Treatment (non-pregnant adult)

- Macrolide - azithromycin OR clarithromycin OR erythromycin




OR




- Tetracycline - doxycycline

Treatment (pregnant or child)

- Macrolide as above (diff dose)


- Tetracycline if benefits outweigh risks
