Atropine Sulfate

Class
Parasympathetic Blocker (Parasympatholytic or Anticholinergic)
Atropine Sulfate

MOA
1. Inhibits parasympathetic stimulation by blocking acetylcholine receptors
2. Decreases vagal tone resulting in increased heart rate and AV conduction
3. Dilates bronchioles and decreases respiratory tract secretions
4. Anti-secretory action suppresses sweating, lacrimation, and salivation

Indications
1. Symptomatic bradycardia
2. Asystole
3. Pesticide poisoning
4. Pulseless Electrical Activity (PEA) less than 60 beats per minute

No absolute contraindications

Side Effects
Cardiovascular: tachycardia, ventricular tachycardia/fibrillation, atrial fibrillation
Neurological: seizures, dizziness, irritability, hallucinations
Respiratory: mucus plugs
Other: increases intraocular pressure, worsens glaucoma, blurred vision
Atropine Sulfate

Precautions
1. Administer supplemental oxygen due to the increase myocardial oxygen demand
2. May cause paradoxical slowing of the heart rate if less than the therapeutic dose is given or dose given too slowly – may also worsen Mobitz I or Mobitz II heart blocks
3. Not recommended in asymptomatic bradycardias due to the possibility of inducing or increasing a myocardial infarction

Interactions
Tricyclic antidepressants, antihistamines, and procainamide can all potentiate the anticholinergic effects of atropine.

Routes / Dosages
IVP, IO, ET, IM

Bradycardia 0.5-1.0 mg IVP/IO or 1-2 mg ET
May repeat dose every 3-5 minutes to a maximum of
3 mg
Asystole/PEA 1 mg IVP/IO or 2 mg ET
May repeat dose every 3-5 minutes to a maximum of 3 mg
Pesticides 2 mg IVP/IO/IM or 4 mg ET
May repeat dose every 5 minutes until patient is asymptomatic or shows signs of atropinization

Pediatrics
Routes /Dosages
All Cardiac 0.02 mg/kg IVP/IO
Minimum single dose is 0.1 mg and maximum single dose is 0.5 mg
May repeat one time in 5 minutes
0.04 mg/kg ET with no repeat
Pesticides 0.05 mg/kg IVP/IO/IM or 0.1 mg/kg ET
May repeat dose every 5 minutes until patient is asymptomatic or shows signs of atropinization
