Mech

Atropine is a competitive agonist for acetylcholine at the muscarinic receptors on the target organs of the parasympathetic system. Heart. Lungs. Salivary glands. Atropine blocks action of the parasympathetic system

Symptomatic bradycardia


Organophosphate/ nerve agent poisoning

Contra

None

Precautions

Atropine may not be effective for bradycardia associated with high level AV blocks. Consider dopamine and pacing first.


Paradoxical bradycardia can occur after slow administration or small doses. Minimum 0.5mg adults. 0.1mg children. Avoid hyperthermic bradycardia

Side effects

Atropine OD causes anticholinergic poisoning. Psychosis, vasodilation, dehydration, elevated temperature, pupil dilation

Interactions

Additive cholinergic effects with other anitcholinergic compounds including antihistamines and TCA's

Dosage

Bradycardia: 0.5 repeated every 3-5 minutes. Max dose 0.04mg/kg



Organophosphate: 1-2mg initially can be repeated every 5-60 min
