Premature Atrial Complex (rhythm, rate, P waves, PR interval, QRS duration)

Rhythm: irregular because of early beat(s)


Rate: usually within normal range, but depends on underlying rhythym


P waves: premature, positive, differs from sinus P waves, may be lost in T wave


PR interval: maybe normal or prolonged


QRS duration: 0.11 seconds or less unless abnormally conducted

Wandering Atrial Pacemaker (rhythm, rate, P waves, PR interval, QRS duration)

Rhythm: maybe irregular a pacemaker site shifts from SA node to ectopic atrial locations and AV junction


Rate: usually 60-100 (if greater than 100; multifocal atrial tachycardia)


P waves: size shape and direction may change from beat to beat


PR interval: varies


QRS duration: 0.11 seconds or less unless abnormally conducted

Atrial Tachycardia (rhythm, rate, P waves, PR interval, QRS duration)

Rhythm: regular


Rate: 101-250


P waves: Atrial P waves differ from sinus P waves with isoelectric baseline between p waves


PR interval: maybe shorter or longer than usual


QRS duration: 0.11 seconds unless abnormally conducted

Atrioventricular Nodal Reentrant Tachycardia (rhythm, rate, P waves, PR interval, QRS duration)

Rhythm: usually very regular


Rate: 150-250 bpm in adult typically 180-200


P waves: often hidden in QRS complex, negative p waves can appear if ventricles are stimulated first, P waves distorts QRS when atria depolarizes before after ventricles


PR interval: p waves are not seen before QRS complex, therfore not measurable


QRS duration: 0.11 seconds or less unless abnormally conducted

Atrio Reentrant Tachycardia / Wolff-Parkinson-White pattern (rhythm, rate, P waves, PR interval, QRS duration)

Rhythm: regular unless associated with aFib


Rate: usually 60-100 bpm if underlying rhythym is sinus in origin


P waves: normal and positive in lead II unless WPW is associated with aFib


PR interval: if P waves are observed less than 0.12 seconds because the impulse travels quickly across the accessory pathway, bypassing the normal delay in the AV node


QRS duration: usually more than 0.12, delta wave may be seen

Atrial Flutter (rhythm, rate, P waves, PR interval, QRS duration)

Rhythm: atrial regular upto 180, ventricular regular or irregular depending on AV conduction and blockade


Rate: atrial rate ranges from 240-300 bpm, ventricular varies depending on AV block usually won't exceed 180


P waves: no identifiable P waves, sawtooth flutter present


PR interval: not measurable


QRS duration: .11 seconds or less but maybe widened if flutter waves bury QRS complex or if abnormally conducted

Atrial Fibrillation (rhythm, rate, P waves, PR interval, QRS duration)

Rhythm: ventricular rhythym usually irregularly irregular


Rate: 300-600


P waves: not identifiable


PR interval: not measurable


QRS duration: 0.11 seconds or less unless abnormally conducted
